If you are able to secure an embossed copy of the form please complete the following steps; 1. Scan and e-mail a copy of the form from a representative of the team to nfatournamentcommittee@gmail.com by March 30, 2026; 2. Bring a physical and embossed copy of the form to registration at the tournament.
If you are unable to secure an embossed copy of the form please complete the form and instruct your Registrar's office to e-mail a scan of the signed form or a digital copy of the form with explicit endorsement and verification of the students listed on the form to nfatournamentcommittee@gmail.com by March 30, 2026. The e-mail MUST come from the registrar in order to substitute for an embossed form.
