Welcome to your School Improvement Planner!

We hope this can be used as a tool to identify the gaps and track the implementation of plans to drive positive change in your building. Please review the suggested process outline below.
If you have any questions please contact your CAO.

Use Tab "Understand & Define"
10
11
★ To Be Drafted Prior To June 2nd ★ 1.Review your school's OSIC key metrics, stakeholder input, any additional school-specific data points. Use to build your SIOT (Strengths, Areas for Improvement, Opportunities, and Threats) chart. Statements should be evidenced by data as appropriate.
12
Ex: Student ownership within math instruction is varied across classrooms: 24-25 math walkthrough data
13
14
2.Define 2-3 Priority Problems to focus on improving for the 2025-2026 school year (We can't do it all! So, what is highest leverage?). These should be aligned to data sets provided and SIOT reflection.
15
Ex: 10% decline in ILEARN Math proficiency, 15% of our student body received an OSS in 24-25, 45% of 3rd graders not IREAD proficient by Spring
16
17
3.Fishbone or 5-Why your priority problems (if applicable) to tease out contributing factors and root causes. Can be very helpful in determining what is highest leverage across problems and make determining change ideas more clear later.
18
Ex: OSS problem may be due to a variety of factors that need to be unpacked such as teacher skill, student engagement, systems of support, etc.
19
20
Use Tab "Theory of Improvement"
21
22
★ To Be Drafted by July 31st ★4.For each priority problem, establish a rigorous and realistic 3 year outcome goal rooted in an OSIC Key Metric. Then, chunk that into your 1 year outcome goal that your planning will center around.
23
Ex: Increase ILEARN math proficiency by 10%, Reduce OSS by 20% (insert actual numbers), 80% of 3rd graders will pass IREAD by Spring.
24
25
5.Define the root causes contributing to the priority problem and craft change ideas you will implement to attain your outcome goal this year. For each change idea, determine how and where the change will be implemented following the question guide of essential change practices.
26
Ex: Support teachers to reach skillful use with EM^2 math curriculum. Start with ILT to build shared knowledge and collective commitments to this work. Scale knowledge in first staff meeting with support of ILT. Finalize individual commitments per context in each PLC.
27
28
6.Chunk your implementation with quarterly aims and define progress monitoring improvement metrics that will show you if what you're doing is working or not. (Crucial to step back here and check for alignment and belief across theory of improvement. "If we do ___ and if we do ___ and if we do ___ then ___ will happen.")
29
Ex: By the end of Q1, each PLC will have defined a goal with commitments. Progress monitoring chunked by PLC rubric scale score improvement (should be directly aligned to change idea). Change vehicles identified as Staff Mtg and PLCs.
30
31
Use Tab "PDSA Planner"
32
33
★ Drafted Qrtrly at Minimum ★7.Use the Outcome goals and Aims that are automatically populated from your Theory of Improvement to detail your actions by month, by vehicle. Consider your theory of action as a whole and what else may be happening week by week that may inform your planning to reach your quarterly implementation and improvement aims. This is set up to support your team as you PDSA by month, by quarter.
34
35
36
8.Define actions to reach quarterly implementatrion aims. Check the box next to each action as completed to track completion. Trends of completion will populate below the planner (starts on row 44) to determine if there are barriers to implementation.
37
38
39
9.Use the SIP monthly reflection portion of this tab (starts on row 53) to step back and reflect regularly with your ILT and/or CAO team on what is going well, what needs improvement, how you know, and commit to next steps revising your plan as appropriate.
40
41
42
Use Tab 'Progress Monitoring'
43
44
★Quarterly or by Benchmark★10.Intended to be used by quarter or by benchmark, this tab will help you monitor progress toward goals using the implementation and improvement aims defined within your Theory of Improvement. You can look across a change idea or across a quarter.
45
46
47
11.Gray cells are meant to be updated by your team to track implementation status using the dropdowns provided. Improvement has space to insert your actual metrics and has dropdowns provided to represent status of progress toward goals.
48
49
50
12.Use the cells by each change idea to hyperlink applicable data sets that you are using to monitor progress.
