Risk
21
Severity (Consequence) x Probability ((Exposure + Occurrence)/2)
22
23
Probability or Severity Risk Mitigations
24
If Initial risk = 5 x 5 (High)
(severity of critical X Probability of Occasional) and we apply 70% reduction to Probability of 5, then
5 - (70% X (5 - 1) = 2.8 (1 is the lowest level of Probability, not Zero)
so the New Probability is
5 - 2.8 = 2.2
Therefore, the new Risk is 5 (severity) x 2.2 (New Probability) = 11 (Serious)
25
Additional Controls
26
If we now have the new risk at 11, and want to add a second control, say administrative with no monitoring of 20%
Probability 2.2 - (20% X (2.2 - 1) = 0.24
so the New Probability is
2.2 - 0.24 = 1.96
Therefore, the new Risk is 5 (original severity) x 2.0 (revised probability with additional controls) = 10 (Serious)
27
28
FSI Calculation
29
So if there is a Severity of Catastrophic (5)
And a probability of exposure is Frequent (5)
And the Probability of Occurrence is Now (0) - has not occurred but is exposure is present
the data may be skewed, therefore:

The calculation is:
5 x [(5+0)/1] = 5 x 5 = 25 (High Risk)
Note: the denominator changed to 1, instead of 2, as Occurrence is no longer applicable
