Some states have implemented their own penalty for not having health insurance coverage. In order to avoid a penalty for non-compliance at the state level, you must bring in a form 1095 A, B, and/or C (your health insurance provider and/or employer must make these forms available to you) or a certificate of exemption if you qualify for one of those provisions. Failure to provide one of these forms will cause you to incur a penalty which will be added to your total tax liability. Please see the "ACA Info" worksheet for more information.
