16 or so on a good week? I usually always cook breakfast and dinner for myself, and sometimes even lunch, but it can be difficult during busier weeks. Lunch is typically the meal that I eat out.

Whole Foods since I work there and get a 20% discount making it just about the same price as your typical grocery store.
Not often. I try not to waste food since it's so expensive.
I usually eat the meals I cook by myself or with my friends/classmates if I bring them to school. If I'm eating out, I'll usually be with at least one other person.
If I'm feeling generous, I'll cook for my roommate.
I don’t have time to cook, Too much effort, Eating out is fun/social, I usually do cook for myself, but these factors definitely play into the times where I'd rather order Uber or go out with friends.
