The Office for National Statistics (ONS) publishes data on people who have died where COVID-19 has been mentioned on the death certificate. This will include people who may not had a diagnostic test. PHE does not hold data on deaths broken down by the method used to diagnose COVID-19. The ONS has published the following test as part of its bulletin on deaths:

“Because of the coronavirus (COVID-19) pandemic, our regular weekly deaths release now provides a separate breakdown of the numbers of deaths involving COVID-19: that is, where COVID-19 or suspected COVID-19 was mentioned anywhere on the death certificate, including in combination with other health conditions. If a death certificate mentions COVID-19 it will not always be the main cause of death but may be a contributory factor. This new bulletin summarises the latest weekly information and will be updated each week during the pandemic.

These figures are different from the daily surveillance figures on COVID-19 deaths published by the Department of Health and Social Care (DHSC) on the GOV.UK website, for the UK as a whole and constituent countries. Figures in this report are derived from the formal process of death registration and may include cases where the doctor completing the death certificate diagnosed possible cases of COVID-19, for example, where this was based on relevant symptoms but no test for the virus was conducted. Our figures also include any deaths that occur outside hospital.

In contrast to the GOV.UK figures, we include only deaths registered in England and Wales, which is the legal remit of the Office for National Statistics (ONS). Table 1 provides an overview of the differences in definitions between sources. From 29 April 2020, DHSC started to publish as their daily announced figures on deaths from COVID-19 for the UK, a new series that uses improved data for England produced by Public Health England (PHE). These figures provide a count of all deaths where a positive test for COVID-19 has been confirmed, wherever that death has taken place, a change from previously reporting only confirmed COVID-19 deaths in hospitals. Figures for Scotland, Wales and Northern Ireland have already 3 begun to include deaths outside hospitals, so this change ensured that the UK-wide series has a shared and common definitional coverage. A statement was published by the ONS which provides more detail of the changes.”

So at the point that providers notify CIW of a death of a service user they cannot confirm that the death was Covid-19 related.

CIW can only therefore provide data on the total number of deaths of people living in care homes or other registered care services.

Of these, 80 had been confirmed with test results.

Two of these UK armed forces personnel have subsequently died. Both personnel were from the Naval Service. Please note, this information is provisional until death certificates are received from NHS Digital and the General Register Office which confirms the cause of death.

Death certification as involving Covid-19 does not depend on a positive test. A doctor can certify the involvement of Covid-19 based on symptoms and clinical findings – a positive test is not required. issuing the Medical Certificate of Cause of Death (MCCD). This
conversation focuses on the reasons for admission, clinical examination
findings, findings from relevant investigations such as swab results,
viral tests, chest X-ray, full blood count, C-reactive protein result and
CT chest findings, and the working diagnosis from the clinical team
looking after the patient. Covid-19 has a characteristic clinical pattern
and when this is present with a positive swab result, and in the absence
of positivity for other viruses, a diagnosis of Covid-19 would usually be
made. The Medical Examiner and doctor would also discuss what
contribution Covid-19 has made to the death. Sometimes it is clear it is
the cause, sometimes it is felt it is more of a contributing cause in
which case it is put into part 2 of the Medical Certificate of Cause of
Death. Sometimes it might be felt that Covid-19 did not contribute at all
(e.g. in a patient who is dying of a terminal cancer) and therefore it may
be omitted from the death certificate despite a positive test result. All
of this process relies on the expertise, training and judgement of the
doctors involved and is tailored to the unique circumstances of each
patient.

required to have Covid 19 somewhere on the death certificate.
This includes all deaths where patients have suffered the effects of Covid 19, where a positive swab has been returned and their  Covid symptoms have directly contributed to the death. Covid 19 should then be written in Part 1 of the Medical Death Certificate. If a patient has had a positive swab in the recent past but has died of other causes then it is appropriate for Covid 19 to be placed in part 2 of the certificate, as a contributory factor. However, some patients are never tested for Covid 19 due to emergency situations etc or they have had a negative swab. These patients are then diagnosed with Covid 19 on a differential diagnosis. They are, diagnosed by particular changes in the lung tissue, appearances of which are specifically characteristic of Covid 19. The diagnosis of Covid 19 is given when these changes are seen alongside changes in blood results, known as Lymphocytopenia.
Other characteristics include, Type 1 respiratory failure. The whole picture leads to the differential diagnosis. Therefore these collective characteristics are specific to Covid 19 and should be written on the Medical Death Certificate in these cases.

The weekly data published on 10 December 2020 showed 43,537 deaths of
patients who have died in hospitals in England and have tested positive
for COVID-19. The daily total deaths publication on the same day showed
(separately) a further 2,092 deaths where a positive test result for
COVID-19 was not received but COVID-19 is mentioned on their death
certificate.
The weekly data set shows on Table 3 that of the 43,537 patients that had
tested positive for COVID-19, 41,683 patients had one or more pre-existing
condition and 1,854 patients had no pre-existing condition. We do not
publish data on pre-existing conditions for the patients with no positive
test result for COVID-19.

If a person is diagnosed as dying from Covid 19 but has no
'pre-recorded' pre-existing conditions and the rules for the Medical
Certification for Cause of Death have been changed to allow Cause of Death
to be ruled in favour of Covid 19, this then allows for a huge margin of
error from fact. In addition to this the post-mortems of said people are
not allowed, further bringing into question the 'facts' of what these
people died from. We know from work that was carried out in Italy that
over 90% of post-mortems carried out on people who had died 'from' covid
were found out to have undiagnosed pre-existing conditions. So, how can
the NHS say with confidence that these people did not have pre-existing
conditions. Should this not say no 'pre-recorded' pre-existing conditions
for simple transparency purposes, or would they be more suited in your
'unknown pre-existing conditions' column, as that's were it would seem
