4		Your Name	Enter your full name as it appears on legal documents or trademark applications
5		Address	Address must match the business address in your Amazon Seller or Vendor account settings

7		Admin Email Address	Must match the admin email on your Amazon Seller or Amazon Vendor account.

10		Business Incorporation Status	Know whether your business is incorporated.
11		Phone number	You will need to have immediate access to the phone number you enter to receive an Amazon verification code

13	Brand Eligibility
14		Do your products and packaging have a permanently affixed brand name and logo?	You must have your logo affixed to either your product OR your packaging in order to qualify for brand registry. If your answer to this question is "no" Amazon will not allow you to complete the registration process.

17	Brand & Trademark Information
18		Brand Name Trademarked	If you do not have your brand trademarked, you will not be able to get your brand registered on Amazon.
19		Brand Name to be Registered	Make sure your brand name to be registered is exactly as it appears on your Products, Product pages, and Trademark information
20		Registered Trademark	Must match the name on your trademark
21		Trademark Serial Number	If you're not sure of your serial number, you can search it on US Patent and Trademark website: https://www.uspto.gov/trademark

23		Images of Brand Logo
24		Images of Brand Logo (optional)	After completing your submission, Amazon may ask you to send them images of your brand name/trademark on your product or product packaging. While this is no longer a requirement during the registration process, some of our clients have been asked to provide this.
25		List of Product Categories You Sell This Brand In	You will choose from a pre-definied list of Amazon product categories. See second tab for list of categories. You can select multiple categories from the list.

29	Trademark Lawyer Contact Info (Optional)
30		Trademark lawyer name	If you do not have this information, Amazon will be able to find and send this information to you based on the trademark serial number you enter during the registration process. However, to help expedite your registration, you may want to send the lawyer a heads up and ask them to send you the confirmation code that they receive from Amazon to verify the trademark. Giving them advanced notice may help get a response from them faster so you can fully complete your registration. You can also find the lawyer contact info by using the US Patent tool: https://www.uspto.gov/trademark
