3 | Minor Behavior Incident Reports | Minor Behavior Incident Reports are used to document concerning behaviors that are disruptive to the learning environment. Minor behaviors are required to be documented when the student does not respond positively to multiple attempts by staff to address the behavior using evidence-based practices, over time, in the same setting. If additional staff are needed to address the student’s behavior, resulting in the student remaining in the setting, those behaviors will at minimum be documented as a minor, but may be documented as a major, depending on the behavior. Corrective teaching and/or consequences for minor behavior incidents are coordinated by the staff responsible for that setting (i.e. the classroom teacher, or staff supervising non-classroom settings.) | |
4 | Major Behavior Incident Reports | Major Behavior Incident Reports are used to document behaviors that pose a threat to someone’s physical or emotional safety, or that cause a significant disruption to the learning environment. Any behavior deemed severe enough to result in removal from the learning environment will be documented with a major incident report. Minor behaviors that increase in intensity, or persist over time, resulting in a significant disruption to the learning environment, will also be documented. Corrective teaching of replacement behaviors and/or consequences following a major behavior incident are coordinated by school administration or designee. | |

7 | Abusive or inappropriate language | The student uses inappropriate verbal language, such as swearing or referencing concepts inappropriate/ disruptive to the school setting, but the language is not directed at anyone or intended to cause harm or distress, and the behavior recurs throughout the interval/ class period. | The student's language impacts another student with hurtful or abusive verbal messages, either intending to, or creating a serious risk of, causing psychological harm or distress, especially when the language is based on a protected class or status. |

10 | Bullying | Student delivers direct, indirect, or technology-based unwanted, aggressive behavior that involves a real or perceived power imbalance. The behavior is repeated, or has the potential to be repeated, over time. The targeted student may experience one or more of the following: fear of harm to themselves or property, detrimental effect on health, or detrimental effect on performance or participation in school activities, messages that involve intimidation, teasing, taunting, threats, or name calling. To be documented after a founded investigation of bullying activity by administration or designee. Previous BIRs may be recoded following the resolution of such an investigation. Always mark as major. | |
11 | Communication of a threat | Communication of a verifiable threat through any means to endanger students and/or staff en masse. Used when a credible threat of violence is made against many people. I.e. threats of a mass shooting, bomb threat, etc. Not to be used when one student threatens one other student to intimidate or cause a fight. Always mark as major. | |
12 | Combustible related | Student is in possession of substances/objects readily capable of causing bodily harm and/or property damage (matches, lighters, firecrackers, gasoline, lighter fluid). Always mark as major. | |
13 | Defiance, insubordination, or non-compliance | The student refuses to comply with directions from staff within a timeframe that is appropriate for the students developmental level. The action (or inaction) is low-intensity and does not cause an unsafe situation. | The student purposefully and intentionally refuses to follow staff directives. Use this category if the student's behavior is purposefully disrespectful, or the student used abusive/ inappropriate language, but the behavior is directed towards an adult. |
14 | Disrespect | The student, through words, action, or gestures, communicates disdain for another student. For example, refusing to sit near a classmate or purposefully excluding them from an activity. | To be used only when the student's behavior is disrespectful to another student, and is not more clearly described by another category. |
15 | Disruption | The student's behavior results in the pace of instruction or other school activity to slow or be interrupted, but for the most part instruction continued. | The behavior is sustained, prolonged, or so extreme that the regular operation of the classroom, school building, or event is compromised. Also includes instigation or encouragement of others to participate in inappropriate behavior, leading to disruption. If the behavior causing the disruption is better defined by another category, use that category. |
16 | Drug related | Student is under the influence of drugs or other controlled substances, or is in possession of drugs, controlled substances, or drug-related paraphernalia on school grounds and/or at school events. Always mark as major. | |
17 | Forgery, plagiarism | Copying a peer's homework or classroom assignments when not appropriate. | The student presents graded or assessed work as their own when it is not original work, or falsifies records or signatures. |
18 | Harassment | Student delivers direct, indirect, or technology based unwanted, aggressive behavior that involves a real or perceived power imbalance, and is related to the age, creed, disability, family structure, gender identity, national origin/ethnic background/ancestry, political belief, race/color, religion, sex, sexual orientation, or socioeconomic status. To be documented only after a founded investigation of harassment activity by trained administrator or designee or the Office of Civil Rights. Previous BIRs may be recoded following the resolution of such an investigation. Always mark as major. | |
19 | Inappropriate display of affection | Student engages in inappropriate but consensual verbal and/or physical gestures or contact of a sexual nature with another student. For example, two or more people engage in groping, fondling, or other sexualized activity on school grounds or at school events. Always mark as major. | |
20 | Inappropriate location | The student is observed in a location that is off-limits to students without express permission. i.e. a staff-member's office when they are not present, in the kitchen area of the cafeteria, a seventh grader in the 8th grade hallway, etc. | The student is in a location that is off-limits and also presents an imminent risk or danger to the student or others. i.e. an area where chemicals are stored, the catwalk of the theater, the boiler/electrical room, etc., or the student violates a documented safety plan. |
21 | Lying/cheating | Dishonest behavior with minimal impact. | Delivering false or misleading information in the course of a school investigation or staff inquiry. Purposefully cheating during academic work, sporting event, or other school competition/event. |
22 | Physical aggression without injury (level 1) | Student engages in actions involving physical contact (e.g., hitting, punching, hitting with an object, kicking, hair pulling, scratching, etc.) where the offender(s) or victim(s) do not require professional medical attention. Do not include bullying or harassment. Physical contact imposed on another person that is not serious, but is inappropriate and unwelcomed. | |

This does not apply to decision by the school board, juvenile court officer, juvenile court judge, or district court judge. Always mark as major. | |

32 | Technology violation | The student uses technology without permission or to access content or materials that are not illicit or inappropriate, per se, but that are disruptive or distracting to the student's or other's learning. | The student records, and/or shares on social media, images or video of prohibited behavior while on school grounds or at a school event. The content may not have been created on school grounds if the act of sharing it constitutes a significant disruption to the learning environment. Or, the student views or accesses inappropriate, illicit, or otherwise banned content while at school. |
