The U.S. Department of Labors Occupational Safety and Health Administration (OSHA) has cited Apex Roofing and Restoration LLC, and WW Restoration LLC for exposing employees to fall hazards after a 15-year-old boy suffered fatal injuries from a fall at a worksite in Cullman on July 1, 2019. The companies face $159,118 in penalties. The 15-year-old fell through an unsecured area of the roof at Cullman Casting before 11 a.m., falling 35-40. He was pronounced dead at the scene by Cullman County Coroner Jeremy Kilpatrick. The teenager had a Vestavia Hills address. OSHA cited the companies for exposing employees to fall hazards while performing roofing activities without adequate fall protection, and for failing to provide proper training. Although Apex Roofing and Restoration LLC and WW Restoration LLC are listed as separate entities, OSHA cited the companies as a single employer because both share supervision on a common worksite, and have interrelated operations and integrated working relationships. In a July interview with The Times, Alabama Department of Labor Communications Director Tara Hutchison said Apex Roofing, WW Restoration and Cullman Casting did not have documentation of past labor law violations. “Employers have a legal duty to ensure that their employees are protected at all times,” said OSHA Area Director Ramona Morris in Birmingham. “This responsibility includes providing appropriate training and conducting assessments to make sure workers understand hazards, and supplying fall protection to minimize the risk of serious or fatal injuries.” OSHA’s Protecting Roofing Workers booklet explains fall protection strategies employers can use to protect workers performing roofing operations. The agency’s Fall Protection web page offers additional compliance assistance resources. The department’s Wage and Hour Division is also investigating the employers for child labor provision violations under the Fair Labor Standards Act. Under Alabama law, certain jobs are off-limits to workers 18 and under, including roofing. The companies have 15 business days from receipt of the citations and proposed penalties to comply, request an informal conference with OSHA’s area director, or contest the findings before the independent Occupational Safety and Health Review Commission.

Grain-Hosette and Cain were tasked with delivering 50 feet of hose to the top of the burning silo and applying water there. A feed house supervisor tried to lead the men to the fire but did not have the proper respiratory protection, which is required in potentially harmful atmospheres. The supervisor "was overcome by smoke while on the catwalk several feet from the silo." He gave firefighters direction to reach the blaze before retreating, according to OSHA. Cain and Hosette reached an opening atop the concrete structure and began dispensing the water, while those on the ground began planning to rush more liquid through the side hatch door. A deadly blast boomed into the Saturday morning sky.

The Iowa State Patrol said a car driven by Lisa Pearson-West, 50, of Des Moines, was turning northbound onto NE 70th Street from E. University Avenue when she drove in the path of a westbound semi-trailer truck. The patrol said the truck was unable to avoid Pearson-West's car and struck it in the passenger side. Both vehicles left the roadway and ended up in a nearby ditch. The semi overturned onto the driver's side in the ditch. First responders determined the truck driver, 63-year-old Kevin Deronde of Oskaloosa, died at the scene. His passenger, 59-year-old Linda Deronde, was transported to Methodist Hospital with minor injuries and has since been released. Pearson-West also sustained injuries and was taken to Methodist Hospital.

Captain Ringering lost his life while battling a structure fire on March 6, 2019. A section of a roof peak fell 20 plus feet on top of the fire fighters, killing Ringering and seriously injuring three others.

An OSHA investigation revealed that communication problems contributed to the dangerous conditions at the fire. The 911 dispatchers did not inform the fire fighters that the residents of the house had safely escaped/

Godfrey FPD made entry into basement garage door directly under roof peak instead of entering the standard door that was offset from peak. If [firefighters] were on the side of structure less debris would have impacted [firefighters] and potentially could have saved a life and lessened injuries."

On scene organization was also lacking, the OSHA report said. The report said on-scene leaders did not do a full check of the home until after the incident that left Ringering dead and three others injured.

Firefighters struggled to fight the fire due to water pressure issues, but those issues were caused because the fire hydrant was not fully opened, according to the report. The report said firefighters had to keep shutting off one of the hoses so water pressure could build back up and allow firefighters to spray the flames. The report said it took another five turns to open the hydrant all the way.

Captain Ringering came from a family of firefighters, his grandpa was an East Alton firefighter, his dad Larry Ringering worked his way through the ranks and was East Alton Fire Chief then over the Madison County Emergency Management. His brother was also a firefighter for East Alton then Godfrey.

Friends of Ringering described him as always smiling and a happy guy. Ringering was huge into training new firefighters and was a teacher at Lewis and Clark Community College in the fire science program.

After the fire at a press conference Godfrey’s then fire chief Erik Kambarian described Ringering as “gold” saying “everyone wanted to train with him and be on duty with him, so they could learn from him. He said Ringering's efforts often attracted people to the Godfrey Fire Protection District due to his dedication and professionalism.”

At Captain Ringering’s visitation over 1,000 first responders across Illinois, Missouri and as far as Chicago, New York City, Boston and many more showed their respects. During the funeral procession, hundreds of residents lined the streets holding up signs, flags, and other items to show support.

On August 22, 2019, the Madison County board passed a resolution to demolish the house where Captain Ringering fought his last fire.

The company faces $1,591,176 in penalties. OSHA has placed the silicon chemical products manufacturer in the Severe Violator Enforcement Program.

OSHA investigators determined AB Specialty Silicones failed to ensure that electrical equipment and installations in the production area of the plant complied with OSHA electrical standards, and were approved for hazardous locations. The company also used forklifts powered by liquid propane to transport volatile flammable liquids, and operated these forklifts in areas where employees handled and processed volatile flammable liquids and gases, creating the potential for ignition.
A Chemical Safety Board investigation found that the workers were making EM 652, a silicon hydride emulsion to be used as a water repellent. Instructions to make EM 652 warn of the dangers of the production of hydrogen gas when XL 10 is in contact with acids or bases.
The EM 652 was made in a set of two atmospheric tanks that were loosely sealed. Workers would open the top of these tanks during the production process to, among other things, perform visual observations. These tanks had no engineered system to direct flammable gas, including hydrogen, to a safe location.
The building ventilation system likely caused the flammable gas cloud to mix with air and disperse throughout the building.
Finally, the CSB determined that there were no flammable gas detectors or hydrogen gas detectors with alarms to warn workers of the significant hazard. The generation of gas in the emulsions tank could produce foaming; however, foaming does not normally occur during this portion of the EM 652 process. The placement of the main air mover near the EM 652 process further increased the potential explosion danger from flammable gases generated in the emulsions area.

Firefighter Kody Vanfossan responded to a multi-alarm two-story vacant structure fire in Christopher, Illinois. Upon arrival, heavy smoke could be seen coming out of the top floor of the structure. Vanfossan, along with a second firefighter, began an interior attack on the fire. An additional firefighter later called for the two firefighters to exit the building. As the two firefighters started down steps in the rear of the building, the steps collapsed. While the second firefighter was able to make it out of the building, a Mayday was issued for Vanfossan. Vanfossan was recovered from the building and placed into an ambulance where he was pronounced deceased. The staircase was unstable and called for a ladder to bridge the stairs when a full collapse of the staircase occurred. Vanfossan became trapped on second level.

Daniel enjoyed spending time outdoors camping, fishing, canoeing, and playing with his two dogs Gia and Duke. He expressed frequently his excitement for home improvement projects for his residence, family, and friends.

A city employee was found Tuesday morning August 6th in the basement of the heating and cooling plant at O’Hare International Airport.

Someone discovered the body of a 53-year-old Michael O’Malley on the ground near an electric Cushman service cart in the 400 block of West O’Hare Street, according to Chicago police and the Cook County medical examiner’s office.

The Chicago Dept. of Aviation confirmed that one of their employees was fatally injured Tuesday morning at O’Hare’s heating and cooling plant.

Officers responded about 5:55 a.m. and found the man without any apparent signs of trauma but with blood nearby, police said. An ambulance was called and he was pronounced dead on scene.

Autopsy results were pending Wednesday afternoon, the Cook County medical examiner’s office said.

City records show that O’Malley was a full-time electrical mechanic with the CDA. He lived in Jefferson Park on the Northwest Side, the medical examiner’s office said.

Police were conducting a non-criminal death investigation, and were awaiting autopsy results, a police spokesperson said.

At 7:19 a.m. Dec. 30, Grimaldo, 54, was trying to climb onto an elevated platform at the old coal plant at 3501 S. Pulaski Road when he fell 50 feet to his death, officials said.

Grimaldo, of Little Village, was pronounced dead at the scene, according to the Cook County Medical Examiner’s office.

Emergency crews were called around 2:30 p.m. for a report of a man pinned in a large machine at Younger & Sons Manufacturing, 19223 K # 42 Hwy. The sheriff's office says Zachary C. Ritchey of Wellington had been working on a machine when he became trapped. Investigators say other employees were able to reverse the device and free the man, but he died on the scene. Deputies say the victim was an employee of a company that does maintenance on the machines at Younger and Sons Incorporated.
