The following data will need to be reported in the template:

School District Code: Must be a valid district code in the four character format. List of valid district/BOCES codes found here: http://www.cde.state.co.us/datapipeline/org_orgcodes

School Code: Must be a valid school code in the four character format for the specified school district. List of valid school codes found here: http://www.cde.state.co.us/datapipeline/org_orgcodes

District Name: Must be valid district name. List of valid district names on District & School Codes tab below.

Grade Level: Must be a grade level between kindergarten and third grade. All grade levels in K-3 served by the school must be reported.

READ Act Assessment: Select from the dropdown which READ Act assessment is administered for that specific school and grade level.

READ Act Diagnostic Assessment: Select from the dropdown which READ Act diagnostic assessment is administered for that specific school and grade level.

Name of Core Program: Select from the dropdown which core program is used for that specific school and grade level.

Core Program Publication Year: Select from the dropdown which year the core program was published.

Other Core Program if Necessary: If the core program used by a school for a grade level is not listed in dropdown menu, please write in the name of the core program.

Name of Supplemental Program: Select from the dropdown which supplemental program is used for that specific school and grade level.

Additional Supports and Services: If the READ Act intervention supports and services are not captured in the information already provided, use this space to provide a short explanation of any unique intervention implementation practices, supports, and/or services (500 character limit).

PD Plan: If the district uses READ funds and/or Early Literacy Grant money to pay for teacher professional development, include a description of its PD program. If no reporting is required, please enter "No reporting required" on this tab.

The reporting should include:
• Overview of PD plan (e.g., monthly staff PD session, followed by individual teacher-coaching)
• Content of the PD and how it is evidence-based and designed to help teachers support students who are reading below grade level.
