Site Activity Log Instructions
The Site Activity Log is used to record all services provided to Taxpayers.
A complete and accurate accounting of services is necessary to justify program funding from our IRS and AARP sponsors.
Editing Instructions:
The columns marked "Do Not Remove -- Required for All Sites" are mandatory for all service delivery models. Sites using the Drop-Off model are required to use the columns marked "Drop-Off Return." Sites using a Scan model are required to use the columns marked "Scan Model Return." The Drop-Off Return and/or Scan Model Return columns may be deleted if a site is not operating one or both of those service delivery models.
General Instructions on How to Complete the Form:
Use a new Activity Log for each day of operation, enter the activity date(s) and Site ID number (SIDN), Taxpayer's name, and record all services provided to the Taxpayer. Multiple lines may be used for each taxpayer to facilitate tracking e-filed submissions.

Paper Return Tracking
Check () all applicable boxes to indicate the type of service(s) provided.
• Record
Only Paper Filed (Not e-filed) returns for Federal Returns in the appropriate column - Current, Prior Year or Amended.
• Record State/Local Only Return if a State/Local return was prepared WITHOUT filing a FEDERAL return.
Activity Reporting
• Check () Question & Answer Only if assistance is provided WITHOUT preparation of a Federal or State/Local Return; do NOT count appointment taking
• Record Counselor initials and record Quality Reviewer initials.

E-File Tracking

• Scan Files Uploaded to TaxSlayer -- after image files are uploaded to Taxslayer
• Scans Deleted from Chromebook TSO Scans Folder -- after scan files are deleted from the TSO Scans Folder

Enter any applicable comments as described in the comment examples.
Supervisor Tips:
Totals are calculated by counting "1" for each in all Paper Return Tracking columns and for Question & Answer.
Site totals for paper returns and Q&A are reported to your state and to Tax-Aide National Office using the
Program Metrics record in the Portal.
The cumulative totals for the tax season should be entered in the Portal.
