This file contains data dictionary information for the All of Us Research Program survey materials available for research use. These materials include the Basics, Lifestyle, Overall Health, Personal Medical History, Family Health History, and Health Care Access Utilization surveys as well as several surveys released in the COVID-19 Participant Experience (COPE) series. Each tab of the file contains data dictionary information for each survey with the exception of COPE. The COPE tab contains all survey concepts released in the series along with a "Versioning" column to indicate the survey version(s) in which the concepts appear. Data dictionary fields contained in this file are defined as follows:

- Item Concept: Refers to source concepts assigned to each survey item/question. Note, in addition to item concept codes, surveys are also assigned a "name" code, which is included in the data dictionary for reference.

- Form Name: Refers to the survey in which the concept appears. This column is included as a reference.

- Section Header: Refers to introductory/interstitial text that appears for a set of survey concepts in the survey materials that participants view.

- FiIeld Type: Refers to the field configuration for the item (e.g. text field, single-select response option, multi-select response option).

- Field Label: Refers to the item content/display text associated with the item concept.

- Choices, Calculations, or Sliders: Refers to the response option source codes and accompanying display text for items that are formatted as single- or multi-select field types.

- Text Validation Type: Refers to the validation setting applied, if applicable, to text field types.

- Text Validation Min and Max: Refers to the minimum and maximum numeric values allowed, if applicable, for text field types.

-Branching Logic: Refers to the "parent" item response logic that must be met for the item to display. This indicates that the item is a "child" item that was only shown to a subset of participants based on their response to other survey items.

- Field Annotation: Refers to field note describing when and how the All of Us privacy methodology has impacted the way the item appears in the curated data repository (CDR).

- Generalized Codes: Refers to the code that appears in the CDR in instances where an item response option was "bucketed" or "generalized" as a result of the privacy methodology.

The codebooks for the surveys included in this file can be found in REDCap and are ready for immediate import and use in the REDCap shared library.
