This list is extracted from FA lists and includes only those applying for permits in the Derbyshire, Nottinghamshire and Sheffield & Hallamshire FA areas. A few others are included where the player has a known connection to the Chesterfield FC.

The list is transcribed from a photocopy of the original. The photocopy did not include lists for the periods Oct 1921 to July 1922. For some of the period, both applications and results of applications were listed; from March 1923, only results can be found. the last photocopy supplied was for January 1937.

In the ‘Applied to join’ column, “Same” is used to indicate where a player applied for a permit to play as an amateur at his last pro club. The “Outcome” column details whether an application was refused or later withdrawn.

The “CA” column details the County FA of the club in the “Applied to join” column. Abbreviations in the “CA” column indicate the following:
