Template is where you do the work, create as many columns as you have layers in that Tab or take a copy of the Tab, upto you

Dictionary has the description of each option

If you need a value in a lookup field that doesnt already exist then add it on the lookup sheet and it should turn up on the template, you will also need to manually update the Notes on each cell on the Template Tab and manuualy update the Discionary Tab

If you need a new row, feel free to add it to the sheet, but also let me know what you added and why so I can add it to the Master Template
