A small resistor (22 to 100) should help to eliminate noise on the SPI lines, I didn't have the problem but some testers did.

Capacitors ceramic : I used the 2.54mm pin pitch for all ceramic caps but other sizes could fit. I like the MC0805 from Multicomp/Farnell. I was not able to respect the tolerances of the original design but haven't heard/seen much difference.
