This is not a test - it is simply a questionnaire, so there are no "right" or "wrong" answers. For each statement, please put an "x" in the right hand column (under the headings Almost Never, Seldom, Sometimes, Frequently and Almost Always) that most closely represents the extent to which your team adopts that particular behaviour.

Tick one right hand box for every statement. There are 32 statements in all. Once completed, review the "team development diagnostic summary" found in rows 6 through 10. Tuckman's 4 stages of team development are captured here (forming, storming, norming, and performing). If your highest score is in storming, for example, this suggests that your team is in that stage of team development. If you have similar high scores in two adjacent stages (e.g. forming and storming), this suggests that your team is moving from forming to storming. You may wish to get other members of your team to complete this diagnostic and then compare your results.
