Almost Home provides a variety of assistance to people experiencing homelessness and housing instability. In our Homeless Prevention program, we provide rent, mortgage, and utility assistance to keep people stably housed. In our Emergency Shelter program, we house families experiencing homelessness and provide case management to help them attain permanent housing after exiting the shelter. Alternatives to Violence (ATV) provides comprehensive support for survivors of domestic violence, sexual assault, and human trafficking through a variety of critical services. This project focuses on offering immediate safety and temporary refuge for survivors through our emergency shelter, ensuring they have a safe place to escape dangerous situations. Beyond shelter, ATV provides a range of essential services designed to empower survivors to rebuild their lives. This includes case management, counseling, legal assistance, and job training, all of which are critical in helping survivors regain independence and work towards long-term stability.
Our goal is to break the cycle of violence and homelessness by offering access to sustainable housing solutions. Through wraparound services, survivors are supported every step of the way, from finding stable housing to gaining the resources necessary for achieving independence. ATV’s holistic approach includes mental health support and assistance in accessing community resources, helping survivors avoid chronic homelessness and empowering them to regain control of their lives.
This project directly addresses the immediate need for emergency shelter while also supporting long-term solutions for those at risk of homelessness. Contributions to this project support the provision of these life-changing services in Larimer County, Colorado, ensuring that those in need can find safety, stability, and a path to a brighter future.

Contributions supported overnight shelter and daytime drop-in services at The Source and housing in TGTHR’s permanent supportive housing program at 1440 Pine in Boulder and Nest56 in Denver. Both programs provide robust case management, education and employment support, pro-social activities, and behavioral health and substance use counseling. TGTHR has a reputation among youth as a welcoming and inclusive organization that can help them improve their stability and successfully transition to independence.
Most of the youth we work with report an increased sense of personal safety and security once they enter our services. To ensure that each youth’s needs are met, TGTHR staff connect them to our onsite services and make referrals to additional community resources of support. TGTHR’s onsite behavioral health care team meets regularly with youth. Our goal is to ensure that youth know how to advocate for themselves to get what they need.
At TGTHR, we focus on the following core outcomes for youth in our programs: 1) social/emotional well-being, 2) permanent relationships and connections that increase stability, 3) education and employment opportunities, and 4) exit to safe and stable housing. Behavioral health concerns are high among young people, and some youth struggle to secure and maintain employment. We have a weekly employment support group to help those searching for employment and those working to maintain their employment. Activities include updating resumes, registering for online education and/or job training programs, and identifying new employment opportunities. Youth who leave TGTHR programming continue to receive aftercare coaching for at least 90 days.

We anticipate this project will cost $16,000,000 at completion. The Emergency shelter will address a survivor’s immediate safety needs, while transitional housing provides short-term housing and programs that give survivors the time and services, they need to achieve goals for long-term safety and stability.
This trauma informed project will be designed to incorporate both the Emergency Shelter and the Transitional Housing Program in one building yet with separate spaces. There will be at least 12 bedrooms in the Emergency Shelter. It will also include space for wrap-around services including case management and will have rooms of varied sizes to accommodate the needs of singles and families. A portion of these rooms will be developed to include spaces for pets who bring comfort to their families as they all escape violence. This space will also have a Children’s Play Area, office areas for staffing and for individual, confidential conversations, a shared kitchen and dining, laundry, and shared community space for gathering. There will also be shared private or community bathrooms as part of the design.
The Transitional Housing Program will be a separate space in the same building and will include 18 bedrooms with space for families and individuals. It will include space for case management and housing staff members; meeting space for partner organizations to be engaged with our clients; and at least one large meeting space where training can be provided for those in shelter and those in supportive housing. Kitchen, dining room, community gathering space and on-site laundry room are included in the planning. Additionally, administrative offices and trauma informed space for counselors to meet individually and in groups with those fleeing DV will be incorporated in the design.

Gateway’s emergency shelter houses domestic violence victims and their children whose lives are in imminent danger for up to 30 days. Shelter staff ensure victims/survivors are welcomed into a warm, non-judgmental and supportive environment. Advocates attend to clients’ immediate needs, providing food and clothing ( if necessary) in addition to safe and secure shelter, while facilitating access to critical social support systems through case management, housing navigation, referrals, and other supports to help increase self-sufficiency and transition out of shelter. Additionally, program staff provide safety planning, case management and psycho education about DV, and assist victims with securing vital documents, benefits such as SNAP and TANF. Advocates are available 24/7 to assist with evolving needs of clients, and simply to listen. Our child/family advocates create a secure and structured environment for children who reside at Gateway, providing opportunities for them to work through trauma with structured play activities as well as through one on one and group sessions. Our Extended Care Shelter facility offers clients a more independent living situation, while they are working to get back on their feet. Better Tomorrow serves as the administrative agency to our four programs: Advocates of Routt County, Brighter World Child Advocacy Program, COMMA Projects for Violence Prevention, and Queer Futures. The Advocates of Routt County program assists survivors of domestic violence and sexual assault. Services provided to shelter clients include immediate crisis response, in-person safety planning and risk assessment, individual needs assessment, case management, emotional support counseling, goal setting and life skills, employment referrals, housing advocacy, and other personal advocacy. Residents are also provided food, safe shelter, resources, referrals, and education. CBM’s Project Activities are all centered around our Essential Homeless Services and are delivered through both the CBM’s staff of eleven individuals and numerous trained volunteers. Nearly 30% of our staff have both experienced homelessness and participated in our programs. The majority of our volunteers have regular monthly interaction with our homeless clients. Our volunteer base will have approximately 1,655 individual interactions in 2024 and include high school and corporate teams.
Outreach Efforts and Other Activities: In 2024 CBM will serve thousands of homeless individuals, many of which are repeat contacts. Staff and volunteers utilize both the CBM and volunteer vehicles for meal deliveries to reach partnering organizations and our homeless clients throughout Denver and the front range. Meals prepared and served by staff and volunteers are served both on-site and in outreach opportunities.
Case Management, Homelessness Prevention Services and Supportive Housing: CBM staff members lead the way in providing and tracking these important and essential services. Through Peer Navigation trained staff and the HMIS System, CBM’s goal is to provide a pathway from homelessness to stable housing and living. CBM currently has 7 beds on-site and is licensed for 12 beds.

Often the first point of contact for people experiencing homelessness is through our outreach efforts that offer a safe and trustworthy way to navigate and access resources related to housing, health care, and supportive services.
The Coalition’s Integrated Health Services is recognized for its expertise in caring for patients with the most complex needs: chronic medical and mental health conditions. Health care at one of our Federally Qualified Health Centers, offers medical and behavioral health care, substance use treatment, dental, vision, and pharmacy services for over 17,000 patients annually.
Using the Housing First approach, we match people with affordable housing at one of our 20+ permanent supportive housing properties or through partnerships with landlords in the community. Our Homelessness Prevention, Supportive Housing, and Family Support Services support participants in maintaining housing through intensive case management, clinical services, peer support, housing retention services and resource support to promote maximum independence. Additionally, we provide domestic/intimate partner violence care coordination, crisis intervention and response for victims.
Finally, the Employment Services Department utilizes a client-focused approach by assisting participants in developing an individualized employment plan based on their specific employment experience and goals, including providing assistance in removing barriers to employment such as transportation, work clothing, etc.

This 2.12-acre site will offer 24 residential units to support working families transitioning from poverty and homelessness to stability. With a successful track record since 1987, BeyondHome has successfully helped hundreds of families, including 47 graduates since 2016, of which 28 achieved homeownership. Serving around 200 individuals annually, BeyondHome is dedicated to long-term success through affordable housing, employment, counseling, education, and financial stability.
The proposed development at 7955 Allison Way will consist of 24 residential units, distributed as follows:

● 8 three-bedroom, two-story units
● 6 three-bedroom, three-story units
● 4 four-bedroom, two-story units
The site, zoned for residential use, will feature a density of 11.3 dwelling units per acre (DU/Ac). Designed with the well-being of our residents as paramount, the project emphasizes several project priorities based on 3 major guiding principles: safety, connectivity, and support. The project priorities for this development include the strategic placement of 16 garage spaces and 41 off-street parking spaces to ensure safety and convenience for residents. The oldest 22 homes, built in the mid-1980s, have provided housing to more than 170 families. While these units have seen some cosmetic upgrades over the years, they are now due for substantial renovations to improve energy and water efficiency, streamline maintenance, and offer modern, attractive homes designed with a trauma-informed approach.
This renovation project will be executed in phases, in partnership with HomeAid Colorado and Builder Captains leading the efforts. So far, nine of the homes have been completed or are under construction (to be completed by November 2024), thanks to the generous support of Milender White Construction, Lennar Homes, and Thomas Sattler Homes. Each Builder Captain’s goal is to minimize the renovation budget per unit with contributions from their trades and the broader home builder community, allowing BeyondHome to further our mission of helping families escape poverty and homelessness.
BeyondHome is eager to welcome eight new families into beautifully renovated homes this October and plans to renovate the remaining 13 units over the next two years as current families graduate.

1. Transitional Housing: We provide safe housing for working families at a reduced rate. Families pay 30% of their monthly income toward rent while building their careers and their financial stability.
2. WorkForce Development Skills: Residents work alongside their coach to explore livable wage jobs. Outside partners such as Thrive Colorado, assist families in resume building, interview practice, etc.
3. Private Practice Counseling: BeyondHome partners with 12 private practice clinicians. Every individual at BeyondHome has immediate unlimited access to therapeutic services. Additionally, BeyondHome staff are in a continual process of increasing their degree of trauma informed care.
4. Access and time to earn degrees: BeyondHome provides the time, space, and opportunities for residents to engage various education opportunities including GED, ESL courses, and college degrees.
5. Financial Assistance: If residents experience an unexpected financial hardship such as a hospital bill, or immediate car repair, BeyondHome will provide financial assistance so residents can continue to focus their attention and their funds to plan for the future.
6. Competency rewiring: Survival skills that individuals rely upon in a life of poverty serve them beautifully. Those same skills hinder individuals in a life of safety. At BeyondHome, families participate in various group night courses and regular coaching to learn and engage in skills that lend themself to a life of stability and safety.

Dinner is served in our dining room, or for those who prefer, through our trailer in the parking lot. Sustainable food bags (to-go food sufficient for up to 3 meals) are also available at our trailer.
In addition to providing meals at our location, the Kitchen also works with other agencies in our community to ensure people have access to food. Meals are provided to support Salvation Army’s motel respite program, breakfast at the Loveland Resource Center (LRC), and at other agencies providing food items.
We also help connect our guests with other helpful services to help them secure and/or maintain stable housing.

The Murphy Center for Hope is a day shelter and the hub of resources for people experiencing or at-risk of homelessness in Larimer County. More than a dozen agencies collaboratively operate 30-plus services at the building. Homeward Alliance coordinates those activities and oversees day-to-day operations. People also have on-the-spot access to a continuum of services, including basic needs (mail, lockers, showers, laundry, computer access, etc.) as well as those provided by our on-site partners, including counseling, veteran’s services, substance use disorder treatment, medical/vision/dental and senior services.

Outreach Efforts
Two program areas are supported by HCTC tax credit donations. First, the street outreach team includes 4 staff and two interns. Staff are engaged in building relationships with the unhoused who are living in camps or alone in hidden areas. The team seeks to build trust with the Colorado Springs Police HOT team and people living in homelessness. Also, they connect people with nonprofit resources available in the community, including HPP’s Housing First Program. HPP’s Street Outreach lead is a Licensed Clinical Social Worker and provides compassionate care to the unhoused outside of a clinical environment. In 2023, the street outreach team served 724 individuals experiencing homelessness.
The Housing First team is comprised of 6 case managers, one specifically for Veterans. The case managers keep a close, professional relationship with clients with weekly visits, referrals to medical care, support groups, counseling and much more. The case managers support clients in creating meaningful goals and reaching them. In 2023, Housing first served 135 adults and 42 children. 91.5% of clients remain in stable housing.

Outreach: Every Tuesday night our team of staff and volunteers goes to the motels on East Colfax and builds relationships with the residents. We connect them with the Family Room where they can receive additional services and do follow ups for crisis situations. On Wednesdays we do street outreach to connect with the unhoused population on East Colfax and provide the same services.
Family Room: This is a drop-in space that provides meals, classes, resource navigation, limited case management, and resources (food, clothing, hygiene items) There are 4 days open to the community .

JFS’s Homeless Prevention (HP) program provides financial assistance to protect families facing immediate eviction, ensuring their ability to pay their bills, stay in their homes, and increase their financial stability. This financial assistance can cover past due rent, current rent, utilities, security or rental deposits, and application fees. JFS focuses services on those at greatest risk of homelessness, not those who are the easiest to serve, ensuring they receive quick and effective services through low-barrier policies and procedures. In addition to providing financial support, JFS takes a holistic approach to delivering comprehensive support services, assessing the various needs of the individual or family and providing the appropriate services all from one organization through a range of responsive and inclusive programs to help those in need navigate life’s challenges.
Once the client has been deemed eligible, a case manager will submit a payment request for any rental and utility arrears as well as the current month’s rent and utilities owed in order to avoid eviction and potential homelessness. By paying the full rental assistance needed directly to landlords, JFS prevents clients from having to solicit funds from multiple agencies. During this time, the case manager will simultaneously complete a Housing Stability Plan with the client in order to maintain their housing moving forward.
To increase accessibility, JFS continues to utilize a hybrid service model for its housing assistance programs, offering both in-person and virtual meetings along with phone calls and emails. Virtual meetings have helped reduce barriers to accessing case management, including transportation and childcare, and have allowed JFS case managers to meet more regularly with their clients, regardless of location. By serving multiple needs through collaborative, innovative, and responsive wraparound programs, JFS provides clients with tools to mitigate immediate crises, eliminate barriers to economic stability, and regain long-term self-sufficiency.

Donor contributions support all aspects of our wraparound services to support our unhoused community including securing basic needs like food, clothing, household essentials, transportation, etc. Our case managers work with people we serve to create a treatment plan and assist with securing day-to-day essentials, but also with elements like applications for income, medical or housing benefits, coordination between providers, oftentimes accompanying them to their appointments as an advocate to see them through the entire process. Upon assessment of persons in crisis at our emergency and temporary shelters, a person served is provided room and board with access to a range of on-site treatment services and interventions in a concentrated short-term way that reduces hospitalization and/or criminal justice involvement. Our permanent supportive housing provides autonomy for independent living and supports successful integration in community with little to no turnover in residents. Our staff can provide a direct connection to service providers and treatment for our residents including transportation for easy access to continuation treatment.
WellPower offers a variety of prevention services to support individuals in their recovery and ensure ongoing success. Our Rehabilitation program provides many life skills prevention groups, two resource centers that offer food access, clothing, hygiene and other basic essentials, and supported education and employment services to aid at every level of an individuals’ reintegration to society. For those transitioning into independent living, we offer a payeeship program supporting individuals with their financial goals without jeopardizing treatment plans to ensure a successful transition into independence. Operating 12 residential treatment houses and 9 apartment buildings, our housing services are set up to ensure shelter, safety and treatment through every stage - from crisis to varying levels of group homes with stays ranging from two weeks to 30/60/90 days to 2 years, all the way to permanent supportive housing.

Most participants, ages 18 to 24, have transitioned out of the child welfare system (foster care or department of youth services). We take a strengths-based approach to empower these young people to overcome past challenges and gain the tools to thrive as adults. BTG uses a multi-generational, holistic approach grounded in safe, stable housing and bolstered through wraparound services connecting participants to basic needs, education opportunities, and workforce preparation.
The program leverages a three-year housing voucher provided through the federal Housing and Urban Development’s Family Unification Program (FUP) and administered locally through an innovative partnership between BTG and the Colorado Department of Local Affairs/Division of Housing. The FUP vouchers are intended specifically for young adults who’ve interacted with the juvenile and social services systems. BTG is the only program in Metro Denver that administers the volume of youth specific FUP vouchers in combination with supportive services.
BTG also serves as a designated Family Unification Program’s Family Self-Sufficiency (FUP-FSS) site and can award an additional five years of housing for youth who are employed or attending high school or post-secondary programs – extending the runway for participants to achieve their goals. At the heart of the program are BTG Independent Living and Family Support Coaches who partner with participants to create pathways to success and economic self-sufficiency.

When donors contribute to Mile High United Way’s 211 Help Center, they directly support a variety of critical services for individuals and families in Colorado. One key activity is the finalization of a specialized hotline dedicated to individuals experiencing homelessness or facing eviction. The “Housing Hotline” will offer both English and Spanish options, allowing callers to easily navigate the system and be connected to essential housing-related resources. By calling the hotline, individuals will hear, “Thank you for calling Mile High United Way’s Homelessness Prevention Line, powered by 211 Colorado. Please select 1 for English or 2 for Spanish. A navigator will be with you shortly.” Navigators will then guide callers to relevant services, such as rental assistance and housing support.
In addition to this targeted support, the broader 211 Help Center provides connections to over 7,000 community resources across multiple sectors, including food, housing, childcare, and healthcare. In FY24, the center made over 278,000 referrals and assisted thousands of individuals with applying for essential services like the Supplemental Nutritional Assistance Program (SNAP).
Donors enable 211 to operate this crucial lifeline, ensuring that help is available in moments of crisis, and that resources are accessible in both English and Spanish, with additional translation services for over 240 languages.

Movement 5280’s project provides essential services to the homeless and at-risk youth populations through our Survive, Thrive, and Revive programs. Donor contributions will support a wide range of activities designed to offer immediate relief, long-term stability, and recovery.
Through the Survive program, donors will help fund services at our day center, which is open six days a week, providing meals, hygiene support, and access to essential resources. During extreme weather conditions, we offer overnight inclement weather shelter and serve as a warming site in the winter and a cooling site in the summer. These contributions ensure that individuals have a safe place to rest and stay healthy during harsh conditions.
In the Thrive program, donor support will help us deliver job skills training and employment assistance, empowering our clients to secure stable employment. Activities include
resume-building workshops, job readiness training, and connections with local employers to foster long-term stability.
Finally, the Revive program offers peer recovery services and mental health support to individuals recovering from trauma, substance use, or other challenges. Donors will support
activities such as peer coaching, mental health sessions, and recovery support groups, ensuring participants receive the emotional and psychological care they need.

The following services are offered through the Family Room:
1. The basics. Clients can stop by and access shelter, water, coffee, snack items, a restroom, a plug for their phones, and positive human interactions with our staff. They can rest for an hour, ask for prayer, and have their basic needs seen to.
2. Resource navigation. Staff members help clients navigate available resources and work to connect them to assistance. This includes referring them to resources for transitioning into housing.
3. Support. Peer support specialists meet with clients to connect them to resources and support the clients on their journey because they have experienced a similar journey. Peer support specialists are in active recovery. Clients also access spiritual support through one on one staff prayer and active positive engagement with staff.
4. Giving back. Client “buy-in” engages homeless clients in leadership through service. They agree to assist with the upkeep of the facilities (mopping, sweeping, cleaning the bathroom, etc.) and are allowed to earn privileges through this engagement.

Posada's mission is to create and provide emergency, transitional, and stable housing and supportive services that empower homeless and at risk of becoming homeless families with children, unaccompanied youth 18-24, veterans, and aging adults in Pueblo County to become self-sufficient and valuing each person, believing that everyone has a right to shelter, food, and basic needs.
Posada currently provides emergency shelter for 11 families, transitional housing for 6 unaccompanied youth who are employed or attending school, and 32 units of housing for veterans. We also provide 52 units of low-income housing, 17 units of single room occupancy (SRO) units for homeless and low-income aging adults, and 32 units of low-income housing for clients who many have physical and mental health challenges. In addition to the housing that we manage we also provide emergency hotel shelter nights and rental assistance.

Posada’s priority is to secure the services and housing that people need, whether provided by Posada or through the network of agencies available in Pueblo and the surrounding areas. In doing so the agency provides an integrated system of care to ensure safety and support for the homeless and at risk of becoming homeless through a network of resources where the experts in each area provide services and duplication of resources is avoided. Case Management
Housing Navigation Center staff 1) prevent people from experiencing homelessness by helping them return back to support networks and avoid the homeless services system; 2) assist people in getting back into permanent housing as quickly as possible; 3) help people maintain their housing with supportive services; 4) connect those experiencing homelessness with mental/physical health care, non-emergency shelter, and other long-term resources; 5) provide essential services to help people survive homelessness (overnight cold weather shelter, lockers, laundry vouchers, mail collection, case management services, etc.); and 6) offer space to numerous homeless serving providers to connect with those they serve.Weld814 9th StreetGreeleyColorado80631Urban

Homelessness Prevention Services
A gift to Urban Peak ensures that our programs are fully funded and that we can provide youth experiencing homelessness with all the support they need to exit homelessness. To move from homelessness to safety and stability, youth require intensive, strengths-based case management; access to physical, mental, social-emotional and behavioral health care; education and employment opportunities; 24/7 crisis support; and a place to call home. Urban Peak meets youth where they are at and ignites a spark that puts them on a path toward self-determined, fulfilled lives. We do this by providing a full convergence of services, including:
Outreach Efforts- the street outreach team meets youth where they are – often on the streets they call home. By offering safe conversation, simple food and hygiene materials or clothing to stay warm, the team aims to build trusting relationships with a highly vulnerable population and provide referrals to services throughout the city.
Case Management- all clients have access to 1:1 case management. Case managers provide trusting, supportive adult relationships that help youth heal from their past trauma. They also coordinate access to all other supportive services and work with community partners to provide youth with the resources they need to achieve independence.
Safe Emergency, Temporary or Transitional Shelter- Urban Peak provides youth with 52 beds across two overnight shelters, for minors (20) and for young adults (32). Additionally, we have 84 beds spread out across six “neighborhoods” that act as transitional housing spaces for youth.
Job Placement and Workforce Ready Services- the support services team works with youth to support their education and employment goals, as well as providing physical, mental and social-emotional care.
Homelessness Prevention Services- all Urban Peak’s services are included in this umbrella and a donation supports all of them!
