In consideration of the agreement to permit the above academic team to participate in the Ohio Academic Competition, I, coach, adviser, participant, reader, moderator, or team member, of said team do hereby release and forever discharge the Ohio Academic Competition, and their respective officers, directors, members, and assigns, and each of them, from any and all claims, demands, damages, actions, and causes of action of every name and nature, anticipated or unanticipated, which I may have against any of the aforementioned organizations and persons, which may arise out of, result in, or any manner be connected with the tournament competition in which I participate as a coach, adviser, participant, or team member. Furthermore, I agree to follow all rules of the competition as outlined in the official rules distributed in the registration packet. I understand that any unsportsmanlike behavior on the part of any team member or adviser may be grounds for disqualification from the tournament by the tournament officials who include readers, moderators, and/or the tournament director. Furthermore, I understand that the decisions of the judges and the appeals board are final and may not be appealed. I agree to comply with such decisions. Also, please note that you may NOT be competing at the regional site that is geographically closest to you. The OAC Committee will do its best to assign you to the closest site, but reserves the right to send teams to alternate sites as necessary to equalize the playing fields in the various regions. Should moving teams to different regional locations become necessary, closest placement will be given on a priority basis determined by the date on which registration was received by the OAC Committee.

If payment has not been made by the regionals registration deadline, payment or proof of payment (purchase order or equivalent) must be presented at regionals; otherwise, a late fee of one-half the entry fee will be added to the fee. If only proof of payment is shown at regionals, payment must be made no later than June 30th of that year or a late fee of one-half the entry fee will be added to the fee and an indefinite suspension will be put into place against the offending school until payment is made.

If this registration form is submitted, and contact is not made to withdraw from the field before the end of Regionals registration (March 21st), the registering team is responsible for the $120 registration fee even if they pull out before the OAC Regional competition.
