Add in all UTM information in columns A through F. Do not edit column G.

Stick with lowercase, use dashes and stay consistent

Use a "-" if the field is not used. Do not leave any field blank.

Destination URL is where the visitor will land. final URL is the full URL with the UTM parameters added. Use this in your marketing.
