If the insurance policy includes a deductible or self-insured retention, the application, attachments, and the appropriate fees must be submitted 90 days in advance of the license expiration date.

Renewal Fee Waived--Upon request, the Department may waive the fee for a hospice that demonstrates it is a not for profit organization that charges no fees and is staffed entirely by uncompensated volunteers.

Renewal Discount$300.00A discount of $300 per hospice shall apply if the same business entity owns separately licensed hospices at more than one Colorado location.This is a maximum discount of $300 for each hospice license renewal.

Renewal Discount$425.00A discount of $425 shall apply if the hospice is deemed by an accrediting agency recognized by CMS and remains in good standing with that organization.To be considered for this discount, the hospice shall authorize its accrediting organization to submit directly to the department copies of all the hospice’s surveys and plan(s) of correction for the previous license year, along with the most recent letter of accreditation showing the hospice has full accreditation status.

Renewal License Application
Inpatient + Outpatient
$7,801.98Hospices with the same ownership and governing body that provide BOTH in-home hospice care as well as inpatient or residential hospice care shall pay the listed renewal fee when
BOTH service models are located in the SAME physical location.
No other discounts shall apply, with the exception of the deeming discount.

Renewal Application
Late Fees Charged
10%Six (6) to twenty-nine (29) calendar days after expiration date, a late fee of 10% of the total renewal fee is due in addition to the renewal fee.Failure to submit a complete renewal application and appropriate fees to the Department by the license expiration date will result in late fees as indicated

50%Thirty (30) to fifty-nine (59) calendar days after expiration date, a late fee of 50% of the total renewal fee is due in addition to the renewal fee.

75%Sixty (60) to eighty-nine (89) calendar days after expiration, a late fee of 75% of the total renewal fee is due in addition to the renewal fee.

Invalidated
If a license renewal application and appropriate fees are not received by day ninety (90) following the expiration of the license, the licensee shall cease operation and submit an initial application and associated fees to the Department in accordance with Chapter 2, Section 2.3.

Change of Ownership$7,765.41The current licensee is required to notify the Department by submitting a Letter of Intent (LOI). An email notification will be sent with information on the CHOW application process. The CHOW Application and appropriate fees must be submitted by the prospective licensee at least 60 calendar days before the change of ownership. Requires prior approval by the Department. A new license will be issued.
