My key design principle is simplicity, if you find you don't need a field get rid of it. In fact try to start with as few as possible. The same goes for priorities, status, etc. Many project managers and project management standards overcomplicate things, then waste time trying to define the difference between a priority 6 and priority 7 next step.

#OptionalSometimes it is either useful or necessary to have tracking number associated with each item. Though often this adds little or no value, so unless you are using it to cross reference somewhere consider doing without it.

Simple description of the screen, module, or component being tracked.
