Instructions: This tracking log must include all products required to comply with the "AIS Requirements." An up-to-date copy of this tracking log must be submitted with each reimbursement requisition submitted to the Colorado SRF program covering, at minimum, all products submitted in previous and through the current reimbursement requisition. The log must document the collection of an AIS certification for each product listed and certification listed. The certifications must be available for submission to the Colorado SRF Program upon request.
