1) Click "File" up above and then click "Make a Copy" from the drop down menu

2) Name your copy "[Affiliate Name] - 2025 AEP Registration"

3) Now that you are on your copy, click the tab below named "2025 AEP Offerings"

4) Fill in the Affiliate Name and Point of Contact on lines 8 & 9 of the 2025 AEP Offerings sheet

5) Scroll through the sheet and select what options you'd like by inputting the amount desired in the corresponding blue boxes

6) Once you are done selecting your options, fill your name in on line 83 indicating you agree to the terms stated below

7) Once you are done selecting your options, click "File" up above while on the "2025 AEP Offerings" sheet, then hover over "Download", and then click on "PDF"

8) Make sure the orientation is set to portrait, then click the blue "Export" button at the top right of your screen

9) After exporting the sheet, email the PDF copy to Tia at tia@stcharlesrealtors.com by 12/1/24. Must be paid by 12/31/24.
