This spreadsheet serves as a template for organizations setting up COVID-19 Mass Testing Sites. Go to File > Make a Copy to create your own editable version. You can edit, add, or delete rows & columns as needed for your specific site. Users should define these fields in a proprietary database or otherwise manage this spreadsheet in a secure manner in order to maintain patient confidentiality and comply with you local patient data security protocols.

Patient Samples

This sheet is a single data table for collecting needed information for each patient triaged for testing through the call centre. Each row represents an individual patient on a specific call date.
If an individual calls on behalf of the actual patient, the patient's details should be recorded, not the caller's.
Each row includes patient contact information, symptoms and risk factors, appointment booking, a unique sample code, and test results. These sections for each row may be completed by different staff over several days.
Several columns populate automatically using formulas to reduce data entry workload and improve analytics. These columns are shaded gray.

Patient Appointments

This sheet is a pivot table of the Patient Samples sheet. As patient appointment details are added to the Patient Samples sheet, this table automatically populates to list them by time. Individual dates can be filtered using the filter in the top left. This sheet can be printed to serve as a list of expected appointments per day.

Appointment Availability

This sheet is a pivot table of the Patient Samples sheet. As patient appointment details are added to the Patient Samples sheet, this table automatically populates to list the number of available appointments available per time slot. The general number of slots per time period can be adjusted in cell E1. This sheet can be consulted to help match callers to available appointments.

Sample Manifest

This sheet is a pivot table of the Patient Samples sheet. As manifest numbers are added to the Patient Samples sheet, this table automatically populates to list the samples associated with a specific manifest by unique ID. Manifest number can be adjusted using the filter in the top left. This can be printed to carry with shipments to ensure that all samples have been collected and delivered.
