1915i Providers may be able to assist community members with housing search assistance. To be eligible someone must: have a substance use issue OR mental health issue, AND be 18 years or older, AND be experiencing homelessness as defined by the Department of Housing and Urban Devleopment AND require assistance with at least two instrumental activities of daily living as a result of homelessness or their health care need (managing finances, maintaining housing stability, managing behavioral health symptoms, managing medication, managing substance use related needs, making decisions, coordinating healthcare needs or services, coordinating transportation)
