Action Plan Purpose

The MCH priority action plan is intended to serve as a tool to plan and describe community-level MCH priority efforts including goals, objectives, strategies, activities, timelines, person(s) responsible and progress reporting information. This Excel workbook contains templates for each MCH priority; you may right-click on any tabs at the bottom that do not apply to your agency and delete those particular templates from your workbook. This format is the same as your existing MCH budget workbooks. Please don't hesitate to contact your MCH Generalist Consultant with any questions. Additional tips are provided at the bottom of this page.

Many fields within the action plan templates are pre-populated for your convenience and to provide the specificity that will enable us to create coordinated, statewide movements in each of the MCH priority areas. Please review the pre-populated information and customize the following fields for your agency and community for submission (along with budgets and HCP planning forms) to your Generalist Consultant by May 18th:

Based on last year's success of using a spreadsheet for the Action Plans, we will be using the same format again this year. Two commands that can be particularly helpful when adding text are the "Wrap Text" command and the "AutoFit Row Height" command (under "Format").

In some cases, strategies are labled "required." When that is the case, please note that the other strategies listed in that particular action plan are optional.

To print: The document is currently set to print on legal size paper.

It should be noted that it is not designed to be a communication tool for community partners. Both the local logic models and forthcoming MCH priority profiles will serve as more effective tools for sharing the MCH priority effort with partners and stakeholders.
