Updates from the Community Action Lab with NCHA opened the discussion which led to ideas about a database or communication platform where we could have access to information of services that people are accessing prior to the current interaction; this could help identify their needs, locate unstable households that need monthly assistance, etc. We talked about using the 2-1-1 database, where needs are recorded each time a household calls in, or using the Touch Phase system used by the fire and emergency response units currently involved in the Action LabReview of outreach field trips to Longmont and Fort Collins

Homeless Gear: Program aimed to provide necessary survival gear to those on the streets; much more utilized and important in the winter; no case management or services offered outside of gear and referral to the Murphy Center, Fort Collins Navigation Center

We also reviewed and discussed the diversion tool used in the homelessness response system currently; we highlighted where the language may need to be changed or adjusted for Weld County and for specific populations including those that are being trafficked or those that do not consider themselves homelessOutreach Fort Collins: aimed at intercepting disruptive behaviors in the downtown area only; available as a resource for merchants, community members, police etc. when someone is acting inappropriately; staff comprised of director and two outreach workers that build relationships with folks and refer to services; have materials for those needing services and for the merchants, police, etc. for reasons they could/should contact Outreach Fort Collins; SummitStone (mental health provider for Larimer County) donated a staff person to the outreach team.

We discussed what the system should look like (collaborative) and who else should be at the table in order to create a full understanding of the current system and where the gaps exist that we need to addressHOPE: Provide dinners to Longmont each night; stops at specific locations and fills the gap as breakfast and lunch are offered everyday through the OUR center; no case management

Community Action Lab Updates (CAL): North Colorado Health Alliance is working to fund a coordinator position for the CAL and hopes that Care Coordination and CAL will work together as opposed to duplicating outreach methods. NCHA is working to create a team of care managers that are not tied to funding like the current staff who are limited by Medicaid. This group will help identify people that are at risk of homelessness to help prevention strategies and the Coordinated Assessment and Placement System (CAHPS) will take care of those that are identified after they have begun experiencing homelessness.
