Speech Therapy (ST) services that may be provided under the DSC Program include: i. Evaluation of speech, language, voice, communication, auditory processing, and/or aural rehabilitation status, ii. Treatment of speech, language, voice, communication, and/or auditory processing disorders, including aural rehabilitation. Services may be provided on an individual or group basis, ili. Laryngeal function studies, iv. Evaluations or treatment of swallowing or swallowing dysfunction, oral function for feeding, and v. Aural rehabilitation following cochlear implant, including evaluation of aural rehabilitation status and hearing, therapeutic services, with or without speech processor programming. b. The provider of ST services shall be a qualified provider and be licensed as follows: i. ST qualified provider shall meet the federal requirements of 42 CFR 440.110, and services may be provided by the following professionals within their scope of practice: 1) A qualified Speech-Language Pathologist (SLP) licensed by the ADHS, or 2) A SLP who has a temporary license from ADHS and is completing a clinical fellowship year. 3.1 General Criteria
Providers must meet the qualifications specified in this policy in order to be reimbursed for Florida Medicaid speech-language pathology services.

3.2 Who Can Provide
Services must be rendered by one of the following:
- Speech-language pathologists licensed in accordance with Chapter 468, Part I, F.S. who meet the requirements of 42 CFR 440.110
- Speech-language pathologists provisionally licensed in accordance with Chapter 468, Part I, F.S. who meet the requirements of 42 CFR 440.110
- Speech-language therapy assistants certified in accordance with Chapter 468, Part I, F.S. and working under supervision as defined in section 468.1125, F.S.

Speech-language pathologists may supervise speech-language pathology students while providing services to a recipient. The speech-language pathologist must direct the care, make all clinical decisions, and be responsible for the recipient’s assessment and treatment.

The licensed therapist must be present for the entire treatment time and may not treat another patient or engage in other tasks while supervising a student.

For Medicaid payment purposes, a qualified speech-language pathologist is licensed and in private practice independent of the administrative and professional
control of an employer such as a physician, institution, or rehabilitation agency. Licensed speech-language pathologists in an independent group practice are
eligible to enroll. Speech-language pathologists in other states are eligible to participate if they are licensed in that state and meet the Medicare criteria for
enrollment.
Speech-language pathologists who provided services to Medicaid members who are also Medicare beneficiaries must be enrolled in the Medicare program.

The following are required for all individual providers:
1. CAQH Form
2. Licensure in the state from the state where you practice.
3. Professional liability insurance that covers the provider or provider group (minimums based on state – example, Ohio providers require 1 million occurrence and 3 million aggregate and Indiana providers require 250,000 occurrence, 750,000 aggregate).
4. Education information – for doctors, the education for the specialties they are practicing in should be added on the education page in PNM. For other providers, the highest level of education should be added on the education page in PNM – for example, information about the Master of Nursing for nurse practitioners.
5. Malpractice claims history – disclosure of all malpractice claims is required, whether pending, closed, or settled.
6. Work history – five years of work history is required. If a provider recently graduated and became licensed, work history begins at the time of licensure – in these cases, five years of work history are not required. Any gaps in employment over six months should be entered on this page.

Licensing Requirements

(1) The Division enrolls only the following types of providers as performing providers under the Speech-Language Pathology, Audiology and Hearing Aid Services program:

(a) An individual licensed by the relevant state licensing authority to practice speech-language pathology (SLP);

(b) An individual licensed by the relevant state licensing authority to practice audiology; and

(c) An individual licensed by the relevant state licensing authority for “dealing in hearing aids” as defined in Oregon Revised Statute 694.015.

(2) The Oregon Board of Examiners for SLP and Audiology licenses and the Division recognizes services provided by:

(a) Conditional Speech-Language Pathologists; and

(b) SLP Assistants.

(3) Services of graduate SLP students, furnished under a Conditional SLP License:

(a) Shall be provided in compliance with supervision requirements of the state licensing board and the American Speech-Language-Hearing Association;

(b) Shall be compliant with applicable record and documentation requirements (see also Oregon Administrative Rules in chapter 335, division 010); and

(c) Are reimbursed to the licensed supervising speech-language pathologist.

(4) The Division shall not reimburse for services of a licensed speech-language pathologist while the pathologist is teaching or supervising students in SLP.
