Please note, completion of the referral form, does not guarantee admissions. Prior to intake, the exiting provider must provide proof of a “negative” COVID-19 test within the past 48 hours. Upon arrival, the client must be screened and cleared for entry by on-site clinical staff (Boston Health Care for the Homeless Program). 2) Walk-Ins: Prior to entry into the building intake, the client must provide proof of a “negative” COVID-19 test within the past 48 hours.
