Each Preliminary Plan Design is Custom designed for your client. The information requested on the Preliminary Plan Design worksheet is the minimum information required for us to prepare a plan proposal. It is essential that data is Complete and Accurate. To minimize data entry efforts, Preliminary Plan Design data must be submitted electronically.

1) Company Info

2) Ownership Interests

If the owner or his wife is owner/stockholder in another business, other data may be required. Businesses under common control require special testing to determine eligibility and participation requirements. Be sure to deal with the question and document the answer. List other details on Notes

3) Business Entity

If a Sub S Corporation, Partnership, LLC or LLP - please include the owners Schedule K income in the appropriate column on the Employee Census Data page. If a Sole Proprietorship, Schedule C data will be required for the past 3 years.

4) Other Retirement Plan(s)

If the owner has an existing qualified plan, information about the plan is required. A copy of the Summary Plan Description (SPD) and all pertinent information will expedite your proposal request. If the client has an existing 401(k) plan, the Census Data 401(k) information must be filled out.

5) Planning Objectives

The owner’s objectives and expectations are necessary to generate a plan which will meet the owner’s goals. Prioritize the owner’s objectives by numbering them from 1 - 6

6) Advisors

List the names and phone numbers of the Advisors and/or ‘Decision Makers’. Make note of anyone who could influence the owner’s decision. Knowing the audience allows us the opportunity to prepare the appropriate backup documentation.

7) Other Defined Benefit Plan

If yes, provide plan information: plan summary, recent Schedule B of 5500

8) Contribution Budget

This is the affordable budget that the client wants to contribut. This budget number should include contributions to an existing plan. Note: “Max Amount” is not acceptable.

Census Form

The Census form is mostly, self-explanatory. Actual (W2) Earnings must reflect the reported, prior year W2 wages. Hours Worked should be the actual hours worked the prior year and may be documented as > 1000 or < 1000 hours. Key Employees are employees that the owner may choose to identify as possible 412(i) participants.
