Each club has a tab (see below). Please place the event(s) for your club on the appropriate tab. Each row of the sheet is for a separate event. If your club is hosting 2 events there should be 2 rows filled out completely. Also, each event should have an event description paragraph next to the clubs/projects name.
Everything placed on this document needs to be approved by your advisors!
AS SOON AS YOUR EVENT IS APPROVED please begin to fill this document out (the writing team will be extremely thankful for that!). You will only be able to fill out the blue headings before your event, green headings are measured DURING your event. See the example below as to how to fill out your club's tab.
Deadline for the blue headings is ________for January/Early February Events) and 1 month in advanced for subsequent events.

As part of the 2015 Script Your Future Campaign, students from the Ambassador Program will be giving a presentation on the importance of medication adherence to the Gannon physician assistant students. This event will take place on Friday, February 13th from 10:00am to 11:00am at Gannon University’s Morosky Academic Center. Giving the presentation will be five LECOM students alongside Dr. Rebecca Wise, professor of pharmacy practice. After the presentation, Dr. Wise and the LECOM students will be available for questions. To conclude, a pledge will be passed out giving the PA students an opportunity to commit themselves to a career of medication adherence promotion.
|Our goal is to spread the word of the imporatnce of medication adherence to fellow future medical professionals||This event will be held at Gannon University alongside their Physician Assistant Students||Physician Assistant Students and their future patients||Medication non-adherence along with examples from CV, diabetes, and pulmonary disease state||We will take a tally of the number of students who attend the presentation, as well as the number of signed pledges||6 volunteers (1 professor, 5 students) 1 hour each||25 pillboxes, 30 wallet cards, 47 pledges||47 future providers equating to thousands of future patients||0||47 student/provider pledges signed||0||0||This is a new grading REQUIREMENT this year!
