In brief, Faiza Ahmed killed herself by stepping in front of a train, following contact in the two days leading up to her death with the Metropolitan Police Service, the London Ambulance Service and the Poplar Job Centre. Fuller circumstances are detailed in the narrative conclusion by the Senior Coroner: "This has been an inquest on behalf of Our Sovereign Lady The Queen by me, Mary Elizabeth Hassell, Senior Coroner for Inner North London, touching the death of Faiza Hassan Ahmed who died on 7 November 2014 at Westferry Docklands Light Railway Station, Limehouse Causeway, London.
The jury has made a narrative determination as follows.
Faiza Ahmed had been diagnosed with mental health issues associated with alcohol and cannabis use that put her at risk of suicide. On 6 and 7 November 2014, a number of factors combined to make the risk acute.
On the morning of 6 November 2014, Faiza reported that a man had attempted to rape her in her home. The historical and continuing lack of rapport between Faiza and the police contributed to her not receiving the support she needed following this incident. The period of time elapsing between the sexual offences investigative techniques (SOIT) officer’s first and second attempt to make contact with Faiza meant that she never received specialist support from the Sapphire Unit, nor was she kept adequately informed of what future help was coming and when.
On the afternoon of 7 November 2014, Faiza went to the job centre and made a written statement of suicidal intent. An opportunity to alert the relevant authorities was missed. Shortly afterwards, at 4.05pm, Faiza made a second statement of suicidal intent in a 999 call to the London Ambulance Service (LAS). This information was not conveyed to the ambulance crew who responded, leaving them insufficiently informed to fit their response to Faiza’s needs. Once again, the lack of rapport between Faiza and the police officers and LAS crew on the scene hampered the provision of support to her. Two SOIT officers arrived shortly after the police and LAS, but did not see Faiza. The information they had about the previous day’s report of attempted rape was not adequately conveyed to the police response team and was not conveyed at all to the LAS crew. This left both insufficiently informed to fit their response to Faiza’s needs. Throughout the two days, information held on police databases about Faiza’s history of vulnerability was not relayed to any of the police and LAS personnel who dealt with her, again leaving them insufficiently informed to fit their response to Faiza’s needs.
Shortly after 6pm on 7 November 2014, Faiza went to Westferry Docklands Light Railway Station and, acting alone, died by suicide, stepping off the platform onto the path of an oncoming train.'

I make a narrative determination as follows.
“Michael O’Sullivan took his own life whilst suffering from anxiety and
depression.
The anxiety and depression were long term problems, but the intense anxiety
that triggered his suicide, was caused by his recent assessment by the
Department for Work and Pensions (benefits agency) as being fit for work,
and his view of the likely consequences of that.
His psychiatrist had diagnosed him as having recurrent depression and panic
disorder with agoraphobia. His clinical psychologist had assessed him as
being very anxious and showing signs of clinical depression. His general
practitioner had certified him as unfit for work. None of these doctors had
been asked to provide information to the Department for Work and Pensions.
Mr O’Sullivan was embarking on a course of treatment including:
- antidepressant medication;
- engagement with an employment support officer;
- cognitive behavioural therapy.
The doctor who assessed him on behalf of the Department for Work and
Pensions (a former orthopaedic surgeon) concluded that he was at no
significant risk by working. The assessing doctor did not ask Mr O’Sullivan if
he had suicidal thoughts.”

According to press reports he took his own life in 2010 after he lost an appeal against the finding that he was fit to return to work. He was clinically depressed and had been diagnosed as bipolar. At the inquest into his death the Coroner ruled that the decision that he was “fit for work” had been the trigger for his suicide.

Mr Boamah was found deceased in his own residence on 26 January 2019. Postmortem toxicology was not able to establish the concentration of illicit substances in his blood. However, liver sampling was able to demonstrate the presence of both cocaine metabolites and morphine. The Assistant Coroner's PFD report raised the following matters of concern: "There is a real risk that future deaths will occur where large sums of money are received by individuals who are then placed at risk through unrestrained access to illicit substances. Whilst it is recognised that individuals should not be deprived of funds owed to them, it does not seem that there is a process whereby concerns about such risks can be raised by treating clinicians to the Department of Work and Pensions."

Mr. T was an honours graduate in his forties who had suffered from schizophrenia for some years. He was hospitalised late in 1998 under the mental health legislation for reasons of self-harm. He was starving himself, and his weight had dropped from 16 stone to under 10 stone. During his period in hospital, his benefit was stopped and it was either not restarted on discharge or stopped again soon after. Because we have no access to the transcript, we cannot know for sure. Within a matter of months, the man had starved himself to death with 9p in his pocket. When his body was discovered some weeks later, a scribbled note that lay nearby suggested that he believed that the authorities had killed him, former MP Mike Wood told The Commons.

She had been out of work for just under two years due to stress-related depression when she was assessed for Employment and Support Allowance (ESA). Miss DE did not receive a self-assessment questionnaire and no evidence was requested from her psychiatrist or GP.
