Initial License Application$3,000.00Notify the Department by submitting a Letter of Intent (LOI). An email notification will be sent to you on how to apply. A completed application, attachments, and the appropriate fees must be submitted at least 90 calendar days before the anticipated start-up date.

Renewal License Application$1,700.00Must submit renewal application, attachments, and the appropriate fees 60 calendar days prior to license expiration. If the insurance policy includes a deductible or self-insured retention, the application must be submitted 90 days in advance of the license expiration date.

$1,700.00Assessed if the renewal application is not submitted prior to the license expiration dateFailure to submit a complete renewal application and appropriate fees to the Department by the license expiration date will result in late fees as indicated.

Change of Ownership$3,000.00The current licensee is required to notify the Department by submitting a Letter of Intent (LOI). An email notification will be sent with information on the CHOW application process. The CHOW Application and appropriate fees must be submitted by the prospective licensee at least 60 calendar days before the change of ownership. Requires prior approval by the Department. A new license will be issued.

Revisit Fee$1,700.00May be assessed as the result of a licensure inspection or substantiated complaint investigation where a deficient practice is cited that has either caused harm or has the potential to cause harm to a consumer and the agency has failed to demonstrate appropriate correction of the cited deficiencies at the first on-site visit.
