Quick Tips

• Enter one patient per row on the ‘Patients’ sheet.

• Use the dropdowns in medication columns to pick generic agents. Strength is free text; frequency uses a dropdown.

• BP: Prefer ABPM/HBPM averages if available; otherwise enter clinic BP as 140/90 etc.

• UACR in mg/mmol. eGFR in mL/min/1.73m². HbA1c in mmol/mol.

• Smoking: ‘current smoker’, ‘ex-smoker’, ‘never smoked’ (Y/N approximations acceptable).

• Allergies/intolerances: separate multiple entries with ‘/’ or ‘;’.

• Haematuria: enter ‘Yes’ if microscopy/dipstick positive; else leave blank or ‘No’.

• Heart failure/CVD fields accept free text (e.g., ‘HFpEF’, ‘MI 2018’).

• Diabetes: SGLT2i preferred in T2D with CVD, CKD/albuminuria, heart failure, or high CV risk (unless excluded).

• SGLT2i cautions/contra: eGFR <20, severe frailty/dehydration risk, recurrent genital/urinary infections, active incontinence, prior Fournier’s, prior amputation, DKA history.

• GLP‑1 RA: avoid in pancreatitis history; consider for weight after SGLT2i unless excluded.

• DPP‑4i: avoid in pancreatitis history; dose adjust in CKD (linagliptin exempt).

• Sulfonylureas: hypoglycaemia/falls risk in frailty.

• Pioglitazone: avoid in heart failure or bladder cancer; watch for oedema/weight gain.

• HTN: titrate current agent(s) to max tolerated before adding; avoid ACEi+ARB combination.

• Lipids: offer statin in secondary prevention; primary prevention if QRISK2 ≥10%, CKD, or ≥85y (with judgement); aim ≥40% reduction in non‑HDL.

• CKD referral: ACR ≥70 (unless already appropriately managed) or ACR >30 with haematuria.
