1	North Carolina 4-H Achievement Application Guidelines
2	The NC 4-H Achievement Application may be used to apply for the following awards: 1. NC 4-H Achievement Plan (County Awards: Cloverbud, Green, Bronze, Silver, Gold, Achievement Medal) 2. NC 4-H Honor Club (State Award)
3	The purpose of this document is to highlight the 4-H member experiences through activities, leadership, citizenship, and service. The Achievement Plan and Honor Club Application coincide with one another in terms of 4-H achievements and experiences. This form is meant to be all inclusive of 4-H and non-4-H work, but can be utilized in multiple years for the purposes of applying to each opportunity separately. Please work with your 4-H Agent/Staff to ensure this is being formatted and saved correctly for you.
4	NC 4-H ACHIEVEMENT PLAN
5	DOCUMENT INSTRUCTIONS and REFERENCES
6	Points are earned in five categories: participation, community service, communication, civic engagement, and leadership.
7		Limit any descriptions to one line (Officer responsibility, etc).
8		An entry may include participation on club/local (L), county (C), multi-county/district (M/D), state (S), regional (R), national (N) and/or international (I) levels.
9		Some areas list examples of possible activities/events. The 4-H’er is not limited to these examples.
10		The Non 4-H category under each section is limited to two entries per year.
11		A strong application reflects participation throughout all categories.
12		Only one level may be awarded per year.
13		An activity, event, project, or program may only be used in ONE category.
14		DO NOT add any rows or columns to any section.

16	Totaling Points	Each category will have subtotals which are added for a category total entered in each level.
17		Total Points will include all activities in that category from current and previous years.
18		ALL POINTS WILL BE CALCULATED FOR YOU ON THIS FORM!

20	Select Completed Application Level on the Levels Tab	Cloverbud 5 - 6 - 7		There is no minimum point required for Cloverbud Achievement Applications.
21		Green		Must be 8 years old to begin the Green Level AND 75 minimum points cumulative required
22		Bronze		150 minimum points cumulative required, Green Level Completed
23		Silver		250 minimum points cumulative required, Bronze Level Completed
24		Gold		350 minimum points cumulative required, Silver Level Completed
25		Achievement		450 minimum points cumulative required, Gold Level Completed
26	If this is the first time you are completing this version of the Achievement Plan, and you have previously been awarded a Green Level or higher, insert the minimum points required for that level in the space provided in cell F2 (Point Total Prior to 2022) on the Achievement Plan Levels tab.

28	Show Multiple occurrences over consecutive years of the same activity listed on one row with total number of times in respective column.
29	PRINTING INSTRUCTIONS FOR ACHEIVEMENT PLAN

31	Select "File" in the Menu Bar, scroll to "Print." Once the Print Preview window has opened, you will see at the top right a drop down menu under Print. Select the menu and choose "Workbook."		Under Print: Workbook, you will see a drop down menu for Selection: All Sheets. Click the drop down menu under Selection and be sure that the following sheets are checked:		1. Applicant Info - 2. Participation - 3. Service - 4. Communication - 5. CivicEngagement - 6. Leadership - 7. Totals - 8. Levels	Once you have your sheet selections checked, click the APPLY. Clicking APPLY will close out non-checked selections.	Click the blue NEXT button in the top right, this will open a PRINT SYSTEM window. If you choose to PRINT a copy, find the printer of choice. Electronic versions should be saved as a PDF to be submitted.
32	The Achievement Plan should be printed and submitted to your County 4-H Professional by their deadline.

34	North Carolina 4-H Honor Club
35	Membership in the North Carolina 4-H Honor Club is bestowed upon those 4-H’ers who have exhibited outstanding 4-H civic engagement, leadership, and community service throughout their 4-H career. An Honor Club member continues to provide leadership and service to the total 4-H program throughout life.

39	IMPORTANT: The final application should be downloaded and submitted with photo through Google Form by your County 4-H Professional. Your photo should be a good quality photo (headshot photo).
