Before Clomi: the condition fluctuates between a severe and a very severe state.

After Clomi: with clomi I'm finally reaching the moderate level. I couldn't tolerate nothing without ear protection. I'm also experiencing complete cure of the anxiety. I could also withstand without ear protection airplanes and typing and click on a keyboard and mouse.
Drowsiness is the one remaining side effect soo far. On the normal version I experienced increased libido, increased production of sperm fluid, flashes, increased heart rate, normal blood pressure (i suffer from low BP). On SR I only experience drowsiness and increased libido.I had reactive T but it seems gone for the momen, but it increased my normal old T which I've had since I was 2 years old. I've also had MEM that subsided thanks to Magnesium citrate powder.

Before clomi I have tried B12 folic acid, pre natal supplement, Magnesium citrate powder.

two weeksBefore Clomi: when i started clomi my pains was unberable, I had flight and was taking noofen(phenibut) after I went in my home country i couldn't go outside my room, I was also taking Clonazepam which reduced a bit the pain, but even slight sound was immadiately giving me extreme pain especially facial and stabbing pain. I could not tolerated up to 30db.

After Clomi: It caused me side effects, had increased earache pains, increased stabbing pains, significant drowsiness, sweating alot, headaches.
It caused me allodynia, which is pain from touch, or even pain from slight wind. The pain was in the knees, feet, back, shoulders, face. The pain was a stabbing nerve sensations all over the body. Even with high temperature I was wearing soft shirts and soft clothes to avoid even the slightest wind.I had very stable low frequency Tinnitus 1/10, after Clomi it started extreme reactive and pulsatile tinnitus.

Before clomi I have tried the medications called Noofen (phenibut) and Clonazepam (biggest savior).

Details:
The pain is both aching and stabbing in nature. It worsens with noise exposure but never fully disappears. There is always some baseline pain. The pain can be both immediate and delayed. Some sounds cause instant discomfort, while others result in lingering pain that lasts long after the exposure ends.
Acoustic trauma (cumulative) 25mg twice a day (50mg total per day)Started Summer 2023Before Clomi: Nearly all sounds were distressing. Everyday noises like dishes clattering or objects dropping were painful. Sudden or sharp noises, such as barking dogs or raised voices, were especially intolerable. I experienced ongoing suicidal ideation for many years due to the intensity and relentlessness of the condition.
Now: Since starting clomipramine, I have experienced improvement in all areas:
• Significant reduction in pain
• Better sound tolerance
• Greater ability to go out and function in the world
• Emotional relief and improved mental well-being
My quality of life has improved significantly. With the combination of clomipramine and custom musicians’ earplugs, life is much more manageable. I still avoid noisy environments whenever possible, but I am no longer overwhelmed on a daily basis.

Examples from wife:
- Sounds are much quieter now. "Before if I dropped something he will jump out of his skin 🙁 and that has changed ….. sometimes he doesn’t even realize some noises that I think are too loud"
- "And the main difference is that for example if a dog barked while we were outside in one of our hikes he will be in pain for days ….. that doesn’t happen anymore"
I developed urinary retention as a side effect. To manage this, I reduced my dosage from three times daily to twice daily, which helped alleviate the issue.I grew up on a ranch and began shooting guns at age 10, especially shotguns, without hearing protection. At the time, even my hunter safety course did not mention hearing protection. I believe this early and repeated exposure to firearm noise caused my hyperacusis.

I have been on clomipramine for approximately two years. I do not recall exactly when the benefits began, but it was likely within one to two months.

Before clomipramine, I tried nearly every class of antidepressants, as well as:
• Acupuncture
• Oval window surgery
(non-updated version, this did help)
• Tinnitus Retraining Therapy (TRT)
• Gabapentin (which I continue to take at a dosage of 600mg three times a day)

Tinnitus has not been affected by clomi

Has found custom molded musician's earplugs very helpful

Severe nox in my left ear and moderate nox in right ear. In July 2024 I had a series of horrible setbacks and nox in my left ear became severe. Pain begins 5min-30 min after exposure. The pain goes on for at least 3 hours and in case of very loud sound it can go on for a few days.

Update June 2025: Compared to September 2024, the intensity of the pain has decreased by 30-40%. I can tolerate the sound of car horns much better. But I’m still home-bound. Clomipramine has prevented me from having setbacks. The most common is an ache, (it tends to become burning or stabbing if it gets worse). Loudness is bilateral, Pain is mostly in my right ear (although my left has had some issues as well now. Details: Acoustic Trauma end of August 2019 with minor symptoms. Noxacusis and T really developed from March 2020 onwards, after using headphones whilst WFH.
I have unilateral Noxacusis (right side) only, no Loudness H. Pain is mainly acid, burning, electric shock type
feeling. Numbness, tingling, dull ache. I get electric shocks from pulling at my ear or when
exposed to loud sudden impulse type noises (the closest thing to feeling like a stab for me)
Pain is mainly around my right ear, face, cheek, jaw and back of head. Spreads down to my
right neck, chin, shoulder, back when bad.
I have some constant low level pain in the right side of my face all of the time. There is some
immediate increase in intensity with sound, but most of the pain is delayed, building up and
lasting for many weeks during a bad setback.
Acoustic trauma - very loud noise (>120dB). Left ear was protected with ear defender, right ear was exposed for less than a second.10 mg --> ... --> 75mg --> ... -> 150 mgBefore clomi: I would say I’m a moderately severe case. My tolerance to sounds is pretty low. I use ear
plugs and noise cancelling headphones when outside, but 95% of my time is spent indoors
at home. My lifestyle is highly adjusted to manage my Pain H. The worst triggers for me are
the loud sudden impulse type noises, similar to that of my initial Acoustic Shock. My
tolerance to sounds is generally low. I struggle with being in the kitchen, cutlery, digital noise,
water, etc.

With clomi: Clomi 75mg is providing some pain relief for me. The low level constant pain in the right side of my face is more or less gone. I used to get pain from using earplugs but this has mostly disappeared. I still get some pain from wearing noise cancelling headphones, but it’s less intense and is quicker to go away. I feel that I’m able to tolerate more sound and at slightly higher volumes than when taking Duloxetine, although my tolerance to sound generally remains low. Also my anxiety and general mood is much improved on Clomipramine compared to Duloxetine. My family have noticed I’m much happier and more like my old self. I wouldn’t say it’s a significant or major improvement so far, but things are definitely heading in the right direction. Hoping that increasing the dose to >= 150 mg will improve things further.
Feeling sleepy, taking naps (I never usually nap). Constipation. I have some static in my vision (VSS?) but this doesn’t seem to be any worse with Clomipramine. I sometimes feel more sensitive to light but it’s tolerable. Visual hallucinations when waking in the middle of the night. Dry mouth, especially after exercise. Feeling sick, especially when I’ve not eaten. Sour, bitter, metallic taste that comes and goes. All of these have improved with time. Usually worsen during a dose increase but settle back down.Bilateral tinnitus, worse on my right side, but my T is generally tolerable and does not bother me. With Clomi, my T can sometimes seem slightly louder and “more active”/fluctuating in tone, but it has remained tolerable. No significant worsening.

I believe I have TTTS and the associated thumping in my right ear. With very loud noises,
the sensation is like an electric shock through my right ear. This still seems to be present on
Clomipramine but has improved, i.e., it happens less and when it does happen the sensation
is not as strong as it used to be. Less electric shock type feelings in my ear

I’ve tried different medications to help with the pain and anxiety that Noxacusis presents:
Duloxetine - as mentioned.
Fluoxetine - Useful for anxiety/depression
Carbamazapine - Helps with facial pain (trigenminal neuralgia/nerve).
Meloxicam - for pain but not effective for noxacusis.
Pregabalin - Reduced nox pain but came with more side effects.
Propranolol - Useful for anxiety, no effect on pain.

August 15, 2025: Side effects are mostly gone except for extreme fatigue, anorgasmia and slow gut motility. Tinnitus might even be slightly better now than before I started.

Details: Pain feels like a sharp, piercing sensation deep in my inner ears. I also feel a kind of burning sensation, especially after longer exposure to sound. Sometimes I can tolerate a sound for a while before the pain sets in, especially if the volume is low. But in other cases—especially with sudden or high-pitched noises—the pain is almost immediate and can linger even after the sound stops.

With clomi: So far, the results have been good. I’ve regained some tolerance for digital sounds—I can now listen at low volumes for controlled periods without triggering pain. It’s not perfect, but it’s a meaningful improvement compared to where I started.
In the beginning, I experienced some drowsiness, dry mouth, and mild constipation, but these side effects have eased up over time. Now, I feel more stable and haven’t had any major issues from the medication.I first tried an SSRI after my initial trauma, and it helped to some extent—I was on No-Dep (sertraline) 50 mg. But after a second acoustic trauma, No-Dep stopped being effective. That’s when my doctor suggested switching to clomipramine. I started with 25 mg, then moved to 50 mg, and now I’m on 75 mg. I’m taking the regular (immediate release) version, usually in the evening.

Does not have tinnitus, TTTS, or MEM

I’ve tried supplements like magnesium and ginkgo biloba, but unfortunately, they didn’t make any noticeable difference.

I did notice some gradual improvement over time after the trauma—especially when I avoided loud environments and gave my ears time to rest. But the major turning point came when I started clomipramine.

Details: My noxacusis pain is constant 24/7. It's burning and has an acid like feeling, sometimes stabbing. It's deep in my ears, jaw, cheekbones, mouth, teeth, gums, tounge and scalp.

Details: I suffer from auditory hyperacusis, everything sounded too loud, especially high notes and "S" sounds in sentences. Also pain for months; I had in one ear (left) an acid-like burning sensation that spread around the auditory system sometimes for several days and a sensation of "open wound"; in the right, it came later, not the same pain, a "burning" type pain directly in the ear canal. Sometimes I had facial pain mainly in the jaw, in reaction to noises. I consider my nox, at its worst as mild/moderate 4-5/10. It has decreased with time, confinement and the wearing of permanent earplugs to 0-2/10. I don't know where to place this, but I also suffer from auditory fullness sometimes significant in the form of liquids behind the eardrum and stinging.

The pain was delayed most of the time. It was instant auditory fullness and the pain would come the next day or days after.

Started January 29th, 2025Before clomi: I considered myself a moderate H sufferer.
I couldn't remove my earplugs except in complete silence or very low, soft background noise (nature). I couldn't tolerate any bumps/pops; I couldn't talk to anyone without earplugs (double pro at my worst); I couldn't crumple paper or plastic wrap; I couldn't chew hard things (at my worst); I couldn't hear any digital audio for months because it made me feel worse; I couldn't hear birds, barking, etc.; I couldn't cook/dishwashers, etc., even with earplugs; I couldn't drive with my earplugs.
I had my groceries delivered to my home.

I was confined to my house from July to December 99% of the time. I could only live and communicate with my partner.
I started to tolerate noise a little better in December and went out a little, but it wasn't enough to remove my earplugs in any situation.

After starting clomi:
When I went from 20 to 30 mg, the very next day, I felt like I could tolerate things I couldn't before.
That's also when I started to feel the side effects (sweating and low libido)
Since switching to 30 mg, I've started to feel the first positive effects: The volume has decreased; popping/snapping sounds are less painful; words with S sounds in sentences no longer sound so much like a "broken speaker."
Clomi has taken away what little pain I had left.
I can take my earplugs out at home 90% of the time; I can talk to my partner at a low volume again without pain.
I can listen to some digital audio at low volume without setback or pain (I still have some hearing loss).
I can drive and go shopping, and go out on the street with just foam earplugs. I put in earplugs and can see my friends in small groups.
I'm far from cured, but I'd say I feel 20-30% better than before Clomi.
I will continue to increase it slowly and hope to improve further.
My left ear was my worst ear and the one with the most pain.
Since I switched to 30mg, it has become my best ear... It had hardly ever improved before; it was the right that improved more easily

2025-05-25 Update: I feel 75% better at 150mg (normal version), and I'm gradually returning to my normal life. I still use earplugs only in noisy places like bars or the subway, for example. My tinnitus has also improved

2025-06-27 I've been back at work for a week, and I'm doing well!
Still 80% better, only loud noises/electronic beeps and high-pitched frequencies are still bothersome. I'm staying at 200 mg, but I'm hesitant to increase to 225 mg because the ear fullness symptom still persists too often. It's the only symptom with my T that reminds me that I'm not yet cured.
Current side effects: Sweating, total anorgasmia.
TTTS has disappeared. The hearing fullness is the only symptom still quite present, but it's 50% better

2025-09-17: I stopped taking Clomi in August... What a joy to no longer have side effects... but I gradually relapsed... before I could no longer work, I started taking 37.5mg again 4 days ago. I immediately feel much better, even the tinnitus has calmed down. This medication works really well, I just hope I don't have to depend on it all my life
Fatigue, night sweats, and decreased sexual sensation
The fatigue disappeared over time; the other two are still present but very bearable.
I've had T my whole life, but insignificant to the point of never thinking about it. Since my acoustic trauma, I have bilateral multitonal T, that's significantly higher on my right ear (where I have the most fluid in my ear). T is not reactive, where it's almost imperceptible. T has improved over time (from 8/10 to 4/10).
Clomi didn't affect my T. At first, I thought it might have increased after reading about other people's side effects, but it was mostly psychological, due to fear of this side effect.

Other treatments tried: Benzo (Ativan) to help me sleep because of the tinnitus during the first 3 months after my trauma.
Ambroxol last fall (60 mg/day); it seems to have helped a little with the pain. I haven't taken it for weeks.

I developed TTTS following a major setback (car crash) in my left ear. I suspect it may have developed an acid-burning Nox around my ear.
It subsided over time but persisted. Since feeling better with Clomi, I no longer feel my eardrum vibrating (did Clomi help?).
With the fluids in the ear, I suspect ETD in my right ear.

Stuff like glass plates and metal silverware was hard. Airplanes caused me a lot of discomfort and I would put on earmuffs whenever one flew overhead. Overall I was homebound for about 7-9 months.

I’m at 150mg and I’ve seen improvement in many areas. My pain threshold has improved to some extent so I can hear louder sounds without feeling pain. More significantly though my pain level has dropped by about 80% and and when I am in pain is goes away much faster than it used too. I haven’t been in pain for more than 24 hours in the last month whereas before I was in pain almost constantly. Oct 19th: still seeing improvements. Bumped up to 200mg. January 28, 2025: I've been holding off writing an update about my case, but for those who are interested, I've been having remarkable success on clomipramine, especially after getting to 200mg. Although it's hard to estimate how much I've improved, I'd say at least 80%. I have spent over a month with no ear protection at all in my house, and have had zero pain, except a minor amount after watching a full movie. Currently, I’m able to have no pain around any natural sounds that are 60-70 decibels. I haven’t tested higher but I’m confident that I’ll be fine with at least low amounts of it. I’m in one of the coldest parts of Canada, so I haven’t been outside much but I am also confident that I’ll be fine when I do. Hopefully, the next update will say I’m 90% better. Right now I’m going to continue slowly desensitizing and being patient, but my doctor has offered to let me go up to 250mg of clomipramine so I may do that if I feel my improvement has stalled. Biggest improvements I had were after going to 100, 150, and 200mg.Drowsiness, sexual issues/changes, one week after reaching 150mg experienced "musical hallucinations" which is a rare but known side effect. After 6 months on clomi I now live almonst a normal life. I only use ear pro at my work and at very loud places, like cinemas. I can listen digital sounds with no problema, before clomi it was imposible. Up until 3 days ago, I was feeling really good - about 80% recovered. I could go anywhere without earplugs, like the supermarket, noisy restaurants, or busy streets, and I was pain-free. Unfortunately, I had a setback after a 10-hour car ride without earplugs. The pain in my left ear is back, and my tolerance has decreased a lot. I hadn't had a relapse in months. Clomi is working though. I hope to go back to how I was. Sept 3rd Report: The last time I wrote was during a relapse, but after 2 or 3 days I was feeling fine again. Relapses have been much milder since I've been taking clomipramine. Right now I'm taking 225mg, and I'll stay at that dose for a while. I don't have any pain right now. For something to bother my ear, it would have to be a very loud and sudden sound. During my vacation, I only used earplugs for a 9-hour car ride. The rest of the time I went everywhere without earplugs and had no problems. I can live a relatively normal life.
The first month was very tough, lots of anxiety, depression, chills, insomnia.
