Is a development, or other duplicate sub-domain indexed?Perform a site:domain.com -www search.If it's a development domain apply noindex tags to all pages. After it is crawled block it in robots.txt

Is there 3rd party duplicate content?Run your money/high traffic traffic pages through copyscape.comIf you control the 3rd party content, change it. Otherwise reach out to webmasters to remove the content

Are there multiple versions of pages e.g. http & https, trailing / and non-trailing slash, www and non-www etc? If so, are they properly canonicalized?
Manually check by performing a site:domain.com search301 redirect or canonicalize them
