OverviewContains an application basic overview, project location information, and number of locations / funding information.

Locations CoveredInclude location ID and additional for every eligible location and Community Anchor Institution (CAI) passed. Note: No ineligible locations should be included in the funding request.

Project BudgetDetail for budget information, including the grant request. The budget should capture any and all capital costs, matching funds, locations, contingencies, bond cost (if applicable), and permitting costs. The budget should not include any Operating Expenses. Note, this budget only requires high level financial information. Prior to award, applicants may be required to provide detailed financial information per the guidelines.

Eligibility & Selection CriteriaDescribe proposed technology, speed to deployment & readiness, a summary of applicant experience and confirmation of forthcoming Tribal consent (if applicable).

Project TimelineDetail the start and end dates of listed milestones and responsible parties.

High Level Network Design (Required)The high-level network design should clearly identify:
1) Pre-existing components.
2) Existing approved grant award network design from the original grant award, if relevant.
3) The proposed project expansion plan.

High-level design must include a path layer, all Eligible location points and should include the locations of key network components such as OLTs, splitters, cabinets and interconnection points. The high-level design may be submitted as ESRI shapefiles, geodatabase or KMZ/KML files only.

Proof of Matching Funds (Required)Bank statements or other sources of cash to demonstrate the applicant has clearly met the minimum 20% requirement

Is Tribal Consent Applicable? (Required if relevant)Select N/A, or attach letter of forthcoming consent with application submission.

Supporting Documentation (Optional)Any additional information necessary to provide evidence that the project meets the eligibility criteria and allows the CBO to make an informed decision.
