* Executive & VPs:   Business cards will be charged to Artwork/Printing for your functional area
* Directors:  You must obtain permission from your VP before submitting an order.

Enter your name and certification(s) as you would like them to appear on the business card.  It is acceptable to include certifications from other associations; list your APICS certifications before other association certifications (but after educational level).  Example:  Joyce Lewis, MAEd, CPIM, CSCP, C.P.M.
Chapter Position(s) *
Enter your Chapter Position(s) in the desired order.  Your official chapter title in the standard format will be used. (limited to 3 lines of text) Use "VP of ...." or "Director of..." format  
Instructor Designations or APICS Committee Designation(s)
OPTIONAL FOR BACK OF CARD - Indicate either your Instructor Designations or Committee Designations.  (limited to 6 lines of text) The IDP designations must be in the IDP format.  Example: APICS Master CSCP Instructor
