Event Summary Report
Clubs are required to submit the Event Summary Report by the Wednesday following each competition. Clubs must submit the Event Summary Report for any competition that involves a non-SU team.

Number of SU participants
Include the number of SU participants that were present for the competition.
Number of spectators
Include the number of spectators present (the may be a best guest)
Please list and detail any fights, expulsions, or any conduct violations during competition.
Include the name and detailed description of incident.
Please list any accidents/injuries that occurred during competition.
Include the name and detailed description of accident. This is not a substitution for the accident report.
Never submit passwords through Google Forms.
