Please fill out this form if you wish to attend any of our upcoming Service Trips. Afterwards, one of our team members will contact you regarding the specifics of the trip. If you are sure that you can attend the trip and do not need any additional information before you can pay, please pay on our Home Page by entering the trip cost (or partial payment of $50 or more) and following the prompts where it says Register Now. You do not need to setup a PayPal Account to pay for the trip-simply click on the "Continue" button on the left side of the page. To pay by check or cash, please contact your Chapter Director. Least Adventurous - access to daily showers, hot water, running water, indoor heating, adequate roofing for sleeping, etc. Working with communities that deal with youth delinquency, crime, poverty, health issues, etc. Extremely Adventurous - possibly no access to showers, no hot water, no running water, no indoor heating, outhouses for bathrooms, no adequate roofing for sleeping, etc. Working with communities that lack shelter, clean water, running water, electricity, etc.


Project Pueblo's Refund Policy for Service Trips

Our Refund Policy for Service Trips is the following: 1) Up until 14 calendar days prior to the start of the trip, all Trip Fees will be refundable, minus a cost of a $25.00 administrative fee that will be deducted due to charges imposed on Project Pueblo by PayPal as well as trip costs (such as marketing, trip materials, or books). All refunds will be by check. Please allow up to 10 business days to receive your refund check. 2) Between 14 and 1 calendar days prior to the trip, you can receive a refund of the money paid on the trip minus a $100.00 administrative fee which will cover the above costs as well as the additional costs of obtaining someone to take your spot. However, if you can find someone to replace your position on the trip, the cost will be reduced to $50.00 during this time period.

Project Pueblo’s Transportation Terms and Conditions

On our Service Trips to the Navajo Nation, fuel costs for drivers and any other participants are included in the Trip Cost. However, even though almost all of our Local Volunteer Events are free and we will do our best to facilitate carpools, drivers for Local Volunteer Events will be responsible for their transportation costs (fuel, etc.). Furthermore, on both our Service Trips and our Local Volunteer Events, Project Pueblo and its partners will not be liable for any and all car related incidents that occur while members are traveling to and from an event. The drivers of the cars will be liable for any and all car related incidents according to their respective insurance policies. Project Pueblo Waiver Including Property Loss

1. Assumption of Risk. I understand that I assume all risks outlined in “1a” and “1b” as follows: 1a. Assumption of Risk of Bodily Injury or Death. I understand that participating in the service trip I am applying for (the “Activity”) entails inherent risks of physical injury, including but not limited to, risks that arise from the following activities: - Painting, climbing on steep roofs, ladders, scaffolds, balconies, overhead storage and work heights more than six feet high are common at construction sites. - Roofing, drywall work and other tasks include heavy lifting - Bricks and wood are cut with fast moving blades that cause airborne particles. - Use of personal hand and/or electric tools - There is a high probability for cave-ins when working below grade level. - Servicing equipment while it is in operation, walking under a suspended load. I have been given the chance to ask questions concerning the Activity, and all such questions have been answered to my satisfaction. Having read this form, I am fully aware of the risks and hazards associated with the Activity, and hereby elect to voluntarily participate in the Activity. I voluntarily assume full responsibility for any risks of loss, property damage or personal injury, including death, that may be sustained by me as a result of participating in the Activity, unless caused by the gross negligence or willful misconduct of Project Pueblo, its officers, trustees, agents, employees, members or volunteers (collectively referred to as “Project Pueblo”). 1b. Assumption of Risk of Property Loss. I also understand that participating in the Activity entails inherent risks of property loss, including but not limited to, the risks described on the website of Project Pueblo, Inc. (“http://www.projectpueblo.org,” hereinafter “Website”) and within this Agreement. I have been given the chance to ask questions concerning the Website and the Agreement, and all such questions have been answered to my satisfaction. Having read the Website and the Agreement, I am fully aware of the risks and hazards associated with the Activity, and hereby elect to voluntarily participate in the Activity. I voluntarily assume full responsibility for any risks of property loss, property damage or theft that may be sustained by me as a result of participating in the Activity, unless caused by the gross negligence or willful misconduct of Project Pueblo, Inc., its officers, trustees, agents, employees, members or volunteers (collectively referred to as “Project Pueblo”). 2. Liability Release. In consideration for Project Pueblo allowing me to participate in the Activity, I agree I will not sue Project Pueblo and I release Project Pueblo from any and all liabilities, claims, demands, actions, causes of actions, costs and expenses of any nature whatsoever arising out of any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, arising from the Activity or while upon the premises where the Activity being conducted. 3. Indemnification. I agree to indemnify and hold harmless Project Pueblo from and against any loss, liability, damage or costs, including court costs and attorneys’ fees, that Project Pueblo may incur arising from my involvement in the Activity. 4. Warranty of Physical Fitness. I warrant that I am physically fit and in good condition that will allow me to participate fully in the Activity. I maintain medical insurance that covers me for accidents and illnesses while I am participating in this Activity. I understand Project Pueblo has not made, nor will make, any investigation into my physical fitness or ability to participate in the Activity, and that Project Pueblo is relying on my warranty of my physical condition. I assume full responsibility for payment of medical expenses not covered by my insurance incurred as a result of my participation in the Activity. 5. Emergency Medical Treatment. I grant Project Pueblo permission to authorize emergency medical treatment as it deems appropriate, and agree that such action by Project Pueblo shall be subject to the terms of this agreement. I understand and agree that Project Pueblo assumes no responsibility for any injury or damage that might arise out of or in connection with such authorized emergency medical treatment. It is my express intent that this Agreement shall bind the members of my family and spouse (if any), my estate, heirs, administrators, assigns, and personal representatives. I agree that this Agreement and any claim arising from my participation in the Activity shall be construed in accordance with the laws of the state of California, without regard to its conflict of laws provision. The courts in Riverside County, California, shall be the forum for any lawsuits arising from the Activity or incident to this Agreement. The terms of this Agreement shall be severable, such that if a court of competent jurisdiction holds any term to be illegal or unenforceable, the validity of the remaining portions of shall not be affected thereby. By submitting this application, I acknowledge that I have read this Agreement form, understand it, and agree to be bound by its terms. I further acknowledge that I did so voluntarily and I am at least eighteen years of age.
