University of Kentucky Greek Event Planning Form for IFC and Panhellenic

In planning and implementing social events, members of the Greek community are bound by a number of laws, regulations and policies concerning alcohol and other risk-related issues. The Commonwealth of Kentucky, the University of Kentucky and each inter/national fraternity/sorority have implemented policies and programs that impact directly upon Greek sponsored events. The responsibility for compliance with these policies and the subsequent liability for negligence rest solely on each individual chapter. Through the combination of the University of Kentucky Interfraternity Council and Panhellenic Councils, we have asked that each chapter complete this event planning form so that we may assist your chapter in ensuring the policies of your inter/national organizations are met. These forms serve the purpose of providing a blueprint for minimizing risk in alcohol related events and this form is here to guide you in your planning process. You must complete this form for any chapter event involving the use of alcohol. To register an event: 7 days prior to the event: Online Submit through email this form Submit through email event invitation Submit through email guest list (at the time of submission) **After form submission, Becca or Todd will contact you to confirm your meeting time and they will inform you on where your pick-up location will be. 4-5 days prior to the event: Meeting in 575 Patterson Office Tower Have event planning meeting at previously confirmed time (with FSA Staff and either Exec VP of IFC or VP Organization for Panhellenic) Have hard copy of this form signed by the President(s) of all chapters involved 1-2 days prior to the event: Email finalized guest list to EVP Email all attachments to: todd.montgomery9@gmail.com; rebecca.gower@uky.edu; vinny.sandy@uky.edu; dean.west@uky.edu with the subject of "Event Planning Attachments for (chapter) event on (date)" .
