We are delighted that you are interested in joining our growing ExCEL team of social workers who are committed to improving the delivery of psychosocial-spiritual health care. You may print each page of the application as you move forward using the print function on your internet browser. It is recommended that you complete this application in one-sitting in order to avoid losing information. This application should take under 30 minutes to complete (total of 16 questions and your goal proposal).
Please complete your application thoughtfully and accurately, following the instructions in each of the sections. Please note that part of this application requires you to propose a goal that you will work toward achieving following your attendance. ExCEL promotes the use of the SMART method (Strategic & Specific; Measureable; Achievable & Attainable; Realistic; Time-Framed) for goal development. Please propose your goal in a manner that uses the SMART Method. Further information regarding SMART Goal formation can be found on the ExCEL in Social Work website. ExCEL will assist you in further refinement of your goal throughout the program.
Application Instructions (Please Read the Following Carefully):
This National Cancer Institute-funded professional education program is a collaborative partnership between the City of Hope, the Association of Oncology Social Work (AOSW) and the Association of Pediatric Oncology Social Workers (APOSW). ExCEL is not education purely for education's sake. We have received NCI funding to improve the leadership skills of social workers and we are committed to demonstrating that social workers are capable of improving the delivery of psychosocial-spiritual care.
Participation in ExCEL is a commitment to actively create a program or service designed to more reliably deliver quality care. ExCEL provides competitively selected applicants with free registration, a one-night hotel stay, meals associated with the course, comprehensive course materials and Continuing Education Units (CEU provider approved by the California Board of Behavioral Sciences #2401 for 9 contact hours-no partial credit). Travel costs for airfare and/or mileage will not be covered. Your acceptance to an ExCEL in Social Work course requires completion of an acceptance survey and daily evaluations. Furthermore, you will be held accountable for reporting progress on your program or service goal at 6 & 12 months post-course.
