Please fill out this request form in order to checkout CISD lending technology equipment.  You may complete this form online and then print it out to obtain the teacher project verification signature.  Return the form to your campus librarian or technology teacher.

The CISD filter can track where you have been and where you try to go! Inappropriate attempts made on this device are reported to the CISD filter 24/7 and can result in disciplinary action.
