Optional Practical Training Reporting Form for Change of Address and Employment

U.S. Citizenship and Immigration services requires all international students to report any start of employment or change of employment to the ISSO. Failure to keep your employment information updated may lead to visa status issues. Students may use this form to report any start of employment or change of employment to the ISSO. No additional information is required at this time once you have submitted the form. For students on 12-month Optional Practical Training, remember the following: • Students must work in a field directly related to the academic major on their OPT I-20 • Students must work a minimum of 20 hours a week • Employment during the 12-month OPT period may be paid or unpaid • Students may not accrue more than 90 days of unemployment during the 12-month OPT period • Students must report any change of address within 10 days of moving to the ISSO For students on 17-month Optional Practical Training, remember the following: • Students must work in a field directly related to the academic major on their OPT I-20 • Students must work a minimum of 20 hours a week • Employment during the 12-month OPT period must be paid • All employers during the 17-month must be registered with E-verify • Students may not accrue more than 120 days of unemployment during the 29-month OPT period • Students must report any change of address within 10 days of moving to the ISSO For a list of OPT frequently asked questions from the Student Exchange Visitor Program go to http://www.ice.gov/doclib/sevis/pdf/opt_policy_guidance_042010.pdf
