If any of the bellow information changes it is your responsibility to contact the RHPB Tech Chair to clarify these changes. This form needs to be submitted at least 2 weeks prior to the event date. No equipment request is guaranteed. Tech Chair has the right to cancel or change any contract to protect the equipment and the staff.
