Remember that it is important to have a correctly formatted invitation letter to present to the Embassy or Counsulate. So take care in filling out this form.

IMPORTANT: When filling out this form it is important that you use proper Capitalization and Lower Case lettering when entering your information. For the names and addresses to appear correctly on your invitation letter, take care that you only use Capital Letters when they are appropriate. Do not use abbreviations.
