Impact (part one)
Approximately how many students will be affected by the proposed PD? Explain how you derived your number. Explain how the PD will impact different types of learners.
Impact (part two)
How does the PD foster innovation in teaching and learning? Describe the impact of the proposed PD on yourself and your professional community.
Needs
What classroom, building or district goal, need, problem or opportunity does the proposed PD address? What MPS, professional training or educational experience prompted the interest in the proposed PD/project?
Evaluation
How will you determine whether the project has successfully met your objectives? Describe specific means for evaluation.
Sustainability
At the concluson of the grant, how will the training/knowledge gained be shared among colleagues? How will you determine which colleagues you share with?
Budget
Specify expenses for consultants, materials and supplies, conference/class/training fee, substitutes (please indicate days/hours needed) and other. Be sure to identify the purpose of each expense and its cost.
Other
Will there be funding from additional sources? Are resources needed to sustain or transfer the knowledge after the grant is exhausted? If so, please describe how funds will be obtained.
