Thank-you for your interest in the above study. This study is being conducted by Maria Matthews as part of her MSc in Health Psychology at The University Of Nottingham. The work is supervised by Dr Neil Coulson and has been approved by the universities ethics committee.

This research is interested in your experiences of seeking health information online and what experiences you have had when you have shared this information with health care providers such as your GP.

You will be asked to participate in a short questionnaire which should take no longer than 20 minutes to complete. Questions will be presented in a number of ways. Some may be on a scale for example how much you agree with a statement. Others will be simple yes/no answers. There will be a number of questions where you will have the opportunity to give more information. If you choose to provide this information, some of your statements may be quoted in the research, however, this will be anonymous, and any obvious identifiable data about yourself will be removed. For example, names, dates, and personal information.

- Your participation is entirely voluntary, and you may withdraw your data within 7 days of completion using a password which we will ask you to provide on the next page.

-  Please try and answer all of the questions as honestly as possible, however, if there is a question you do not wish to answer, please skip the question.

- Your responses will be anonymous, and no participant will be identifiable from the research report.

- If you choose to abandon the study midway through, your data may still be used. If you wish to withdraw this data please do so by sending an email to the researcher with your password.

- On completion of the study you will be shown a debrief containing further information about the study and informing you again of how to withdraw your data if you wish to do so.
