~This is the Author Sign-Up form for Round Fifteen of the Small Fandoms Bang on Dreamwidth.  Please fill out all of these required questions.

~E-Mail Addresses are required; we will no longer be contacting people via PM.

~Include the three fandoms you will most likely write in when you sign up.  (If you change your mind down the line you may add/change your fandoms by contacting us at our mod e-mail account.)

~Please write out the full name of the fandom and also include the type of fandom (movie/tv/game/etc) or author name if it is a book fandom to help us out as we will not be familiar with many of the fandoms that Authors sign up for.  [Examples: Blood Ties (tv), The Rundown (movie), Blood and Smoke Series (Tanya Huff).]

~If you plan on writing a crossover, please list both fandoms in the same box, such as Blood Ties (tv)/Jericho (tv).

~If you only wish to sign up for one or two fandoms, indicate n/a in the other space(s).
